Job summary
To work as an
autonomous practitioner, responsible for the delivery of total patient care, working
as part of the practice multidisciplinary team, delivering care within their
scope of practice to the entitled patient population. The Role will encompass
working across four of the embedded sites within the Heart of Lincoln remit.
The Nurse
Practitioner / Advanced Nurse Practitioner / Advanced Clinical Practitioner is expected to, within their professional
boundaries, sees patients on an appointment basis both on the day and pre planned care appointments, provides
care including initial history taking, clinical assessment, diagnosis,
treatment and evaluation of care. They will demonstrate safe, clinical
decision-making and expert care, including assessment and diagnostic skills,
for patients presenting at the practice. They will work collaboratively with
the general practice team to meet the needs of patients, supporting the
delivery of policy and procedures, and providing nurse leadership as required.
Given the current levels of Chronic disease
management within our population we would welcome candidates to apply with
specialist areas in Chronic disease who wish to move into a Nurse Practitioner/ Clinical Practitioner role in General Practice.

Manage
patients presenting with a range of acute and chronic medical conditions,
providing subject matter expert advice to patients
b.
Implement
and evaluate individual specialised treatment plans for chronic disease
patients where trained and skilled to do so.
c.
Identify,
manage and support patients as risk of developing long-term conditions,
preventing adverse effects on the patients health
d.
Provide
advanced, specialist nursing care to patients as required in accordance with
clinical based evidence, NICE and the NSF
e.
Request
pathology services as necessary
f.
Process
and interpret pathology and other test results as required
g.
Provide
daily clinics, delivering patient care as necessary, referring patients to
secondary / specialist care as required
h.
Maintain
accurate clinical records in conjunction with extant legislation
i.
Ensure
read codes are used effectively
j.
Maintain
chronic disease registers in accordance to QOF
k.
Chaperone
patients where necessary
l.
Prioritise
health issues and intervene appropriately
m.
Support
the team in dealing with clinical emergencies
n.
Recognise,
assess and refer patients presenting with mental health needs
o.
Extended
and supplementary prescriber, adhering to extant guidance
