Job summary

We are seeking a skilled and enthusiastic Practice Nurse to join our friendly practice team for 2 to 3 days a week.

Bro Pedr Medical Group has two sites, Taliesin Surgery which is our main site in the University town of Lampeter, and a branch surgery 5 miles away in Llanybydder, both being close to the Ceredigion coast and 40 minutes' drive from Aberystwyth and Carmarthen.

Main duties of the job

Please refer to the attached job description and person specification for comprehensive information regarding responsibilities, qualifications, and expectations.

To assist in and perform routine tasks related to patient care as directed by GPs. Such tasks will include:

  • Assist in and perform routine tasks related to patient care as directed by senior nursing staff and GPs
  • Wound care, ulcer care, dressings, suture removal and minor injuries management
  • Chaperoning and assisting patients where appropriate who are being examined by another clinician
  • Requesting pathology tests, for example urine cultures, swabs
  • Following agreed clinical protocols with referral to senior nurses or GPs as appropriate
  • Maintain general tidiness and cleanliness of nurses and treatment rooms
  • ECGs

Job responsibilities

MAIN RESPONSIBILITIES

a) Assessment of health needs.

b) Planning and delivery of care.

c) Evaluation of direct care for both individuals and groups of patients.

CLINICAL NURSING PRACTICE

Wound management

a) Perform wound care using aseptic technique.

b) Undertake complicated dressings.

c) Suture and clip removal.

d) Assist with minor surgery.

Screening

a) Undertake opportunistic screening - BP checks, height, weight, BMI, urine analysis and smoking status.

Health Promotion

a) Recognise patients readiness to change in supporting their attempts to change unhealthy behaviour.

b) Provide healthy eating advice.

c) Provide smoking cessation advice individually in opportunistic consultations.

d) Provide advice on exercise and increasing activity.

e) Offer advice on safe limits for alcohol consumption.

f) Develop and maintain health promotion material within the practice.

g) Refer as appropriate.

Vaccinations and Immunisations

a) Administer injections under an individual prescription or Patient

b) Group Directive.

c) Ensure safe storage, rotation and disposal of vaccines and drugs.

Infection control

a) Apply infection control measures within the practice according to local and national guidelines.

b) This includes:

i. Universal hygiene precautions.

ii. Collection and handling of laboratory samples.

iii. Segregation and disposal of waste materials.

iv. Decontamination and sterilisation of instruments.

v. Reporting and treatment of sharps injuries

vi. Dealing with blood and body fluid spillages.

c) Assist patients and colleagues in adopting sound infection control

measures.

Emergency management

a) Is competent in anaphylaxis and resuscitation techniques.

Family planning

a) Has a basic understanding of emergency contraception and refers patients as appropriate.

b) Advises on the risk of pregnancy and has a basic knowledge of contraception options.

Womens' health

a) Has a working knowledge of the national and local Cervical Cytology and mammography screening programmes.

b) Undertake cervical cytology sampling.

c) Take swabs for microscopy.

d) Advise on the increased peri-menopausal and post-menopausal risk of osteoporosis and heart disease.

e) Offer health promotion/lifestyle advice to patients.

Male Health

a) Initiate opportunistic consultations with patients to cover issues of male health.

b) Make appropriate referrals.

Diabetes

a) Has a basic understanding of diabetes.

b) Supplies appropriate material for patient education.

c) Is able to manage the emergency treatment of a diabetic patient with hypo/hyperglycaemic attack according to practice guidelines.

Respiratory disease management

a) Has a basic understanding of asthma care: its diagnosis.

b) Checks patients inhaler technique and measures peak flow readings.

c) Has a basic understanding of COPD treatment to support patients.

Cardiovascular disease

a) Has a basic understanding of the pathophysiology of the cardiovascular system.

b) Identify and advise patients on the risk factors associated with cardiovascular disease i.e. BP, lipid, glucose monitoring, family history, smoking, diet and exercise.

c) Work within agreed protocols to empower patients to manage their own cardiovascular disease

d) Make appropriate referrals for review to other members of the team.

SUMMARISING PATIENT NOTES

a) Summarise the medical content from a patients paper records onto the EMIS PCS computerised clinical system following the guidelines set out in the Summarising Protocol on the practice Intranet.

ORDERING SUPPLIES

a) Orders vaccines and other drug supplies as required.

CONTROLLED DRUGS

a) Administers the practice procedure including stock checks, ordering and checking for shelf life.

b) Investigates any stock check discrepancies and identifies any problems.

c) Arranges for any out of date or returned drugs to be properly disposed of.

CLINICAL PRACTICE LEADERSHIP

a) Monitor and maintain the consistency of professional standards set.

b) Take appropriate action if standards are not being met.

c) Participate in nursing and team development
