Hull Family Practice is seeking an enthusiastic and proactive pharmacist to join our existing team of 2 Pharmacists and 2 Pharmacy Technicians.
This is an exciting opportunity to join a well-established and high performing practice to improve the service we offer to patients whilst supporting the GPs and having the opportunity to develop.
Main duties of the job
Acting within
their professional boundaries, within this role you will be working collaboratively as
part of a multi-disciplinary team to clinically assess
and treat patients using their expert knowledge of medicines as part of the
national strategy to address both the public health and social care needs of a
patient.
They will resolve day to day medicine issues and consult with and
treat patients directly. This includes providing extra help to manage long-term
conditions, specific advice for those on multiple medications and better access
to health checks.

large friendly PCN serving 38,000 patients within the East Hull area. We have a team of GP Partners, Advanced Nurse Practitioners, Clinical Pharmacists,
Pharmacy Technicians, Practice Nurses, Health Care Assistants and a large team
of administration staff including receptionists.

purpose built health Centres that we share with a number of other NHS services
and we believe in providing all our patients with the best possible experience
and health outcomes.

JOB
SUMMARY
- Reconcile
medicines following hospital and clinic discharge and work with patients
and community pharmacists to ensure patients receive the medicines they
need post discharge.
- Respond
to requests for acute medication from the general public and manage patients
and health care professionals medicine queries
- Increase
quality and safety of prescribing through mechanisms such as audit
- Manage
adherence to local formularies to improve the quality, safety and cost
effectiveness of prescribing
- Engage
with Humber & North Yorkshire Integrated Care Board (ICB) and NHS England on local and
national healthcare priorities and initiatives
- Implement
drug withdrawals and alerts to improve medicines safety
- Provide
expertise in clinical medicines review and address public health and
social needs of patients in GP practices
- Work
with primary care professional and patients to implement National
Institute of Health and Care Excellence (NICE) other evidence based
guidelines
- Contribute
to multimorbidity reviews and multidisciplinary reviews
- Provide
medicines information and training to practice healthcare professionals
and admin staff
- Review
pathology results for patients on known medicines
- Act as
a source of medicines information for all of the practice team and
patients (e.g. adverse drug reactions, possible alternatives in relation
to stock issues)
QUALIFICATIONS
& TRAINING
- Completion
of a Masters Degree in Pharmacy or equivalent (4 years minimum), and
registration with the General Pharmaceutical Council
- Minimum
of 2 years post graduate experience in pharmacy.
- May
hold or be working towards a postgraduate pharmacy qualification
