Job summary
  We are looking for
a hardworking, enthusiastic individual to join our team as a Patient Care
Advisor (working as a prescription clerk). We are a surgery based in
Warley, Essex with over 13,000 patients. 
We are looking for two part time a job share people, equal to a full time equivalent. The
practice is open 08:30 18:30 every day.
General practice
experience is essential. Use of Systmone software essential. 
A permanent
contract is subject to a successful six-month probationary period.

Processing Repeat Prescriptions
 - Review
 and action requests from patients, pharmacies, and care homes
 
 - Check for
 due reviews, medication synchronisation, and potential overuse
 
 - Flag
 anomalies or concerns to clinicians
 
Liaising with Clinical Staff
 - Work
 closely with GPs, pharmacists, and nurses to clarify medication queries
 
 - Escalate
 requests requiring clinical input or prescribing decisions
 
Medication Safety and Accuracy
 - Ensure
 prescriptions align with current medication lists and formulary guidance
 
 - Monitor
 for high-risk medications, interactions, or contraindications
 
 - Support
 audits and medication reviews where needed
 
Patient Communication
 - Respond
 to prescription-related queries via phone, email, or in person
 
 - Provide
 clear guidance on timelines, collection points, and repeat processes
 
System and Record Management
 - Use
 clinical system Systmone to record actions and update medication records
 
 - Maintain
 accurate logs of requests, issues, and resolutions
 
Supporting Medicines Management
 - Assist
 with prescription switches, formulary updates, and cost-effective
 prescribing initiatives
