An excellent opportunity has arisen for a PCN ANP/UCP to join our friendly practice to help us deliver high quality care. We are looking for someone to work between 30-37.5 hours per week. A 1-year fixed contract to cover maternity leave.
Main duties of the job
The post holder will need to triage the needs of the presenting patient from history taking, assessment, diagnosis, and treatment ensuring any necessary referrals are made while acting within their professional boundaries.
In order to work at this level, the post holder must be a registered general nurse (currently registered the Nursing & Midwifery Council), have a recognised NP qualification (minimum of degree level or equivalent) and be an independent Nurse Prescriber.
Have experience in managing care home patients and care home rounds.
The candidate will ideally have a digital badge or be willing to work towards one.

As an ANP or UCP you will be an experienced clinician and will provide acute same day care to patients registered at the surgery. This includes telephone triage and F2F assessments.
Key Responsibilities
The Advanced Nurse Practitioner works autonomously and is accountable for own professional actions.
Assess, diagnose, plan & implement treatment/interventions and care for patients.
Refers patients to other medical specialities for assessment if deemed appropriate.
Diagnose and manage both acute and chronic conditions
Prescribe and review medication for therapeutic effectiveness in accordance with evidence-based practice and national and practice protocols, and within scope of practice.
Work with patients in order to support compliance with prescribed treatments.
Communicates highly sensitive condition related information to patients, relatives and carers. Initiates the process of diagnosis with patients suspected to have a chronic diseases eg. chronic diseases, within the disease management agenda eg QOF, Contributes to disease registers.
Undertakes annual review of patients understanding and ability to self-manage and medication reviews within scope of competency.
To actively implement safeguarding protocols for children and vulnerable adults and to know who to refer to and how to escalate concerns
To attend care homes and carry out care home rounds managing the residents health.
To work with the PM and PCN CD in PCN work based activitys.
