Job summary
The Role
We are looking for a dedicated and skilled Practice Nurse to join our team. The successful candidate will have experience managing minor illness, as well as working with long-term conditions such as diabetes, asthma, COPD, hypertension, cardiovascular disease, etc. You will work closely with GPs, healthcare assistants and other staff to provide holistic care, education, and support to our patient population.
Main duties of the job
Diagnose and manage common minor illnesses
Regular monitoring, review and care planning for patients with long-term conditions
Deliver vaccinations and immunisations
Conduct health promotion, patient education, lifestyle advice
Complete wound care, sexual health screening as required
Maintain accurate clinical records, participate in audits and quality improvement
Collaborate with multi-disciplinary teams to ensure continuity of care
