An exciting opportunity has arisen to join the SLC Medical Group Nursing team.
We are seeking a Practice Nurse to join our well established and structured practice team. Candidates must be qualified to RGN degree level, be registered with the NMC and specifically hold a Asthma / COPD Diploma or equivalent.
Main duties of the job
You will be an integral part of our Respiratory Management team, delivering expert care to patients with asthma, COPD, and other respiratory conditions. You will also contribute to the wider nursing services within the practice, including:

Conducting respiratory reviews and managing long-term condition clinics

Performing spirometry.

Delivering immunisations and vaccinations

Collaborating with GPs and the wider MDT on patient care plans

Participating in quality improvement initiatives (e.g. QOF, audits)

The post
holder is responsible for the delivery of basic practice nursing service care
to the practice population. Supported by Nurse Practitioners within the
practice, they will deliver care within the boundaries of their role, focusing
upon supporting patients to be healthy, monitoring of long-term conditions,
health prevention and screening activities. They will work collaboratively with
the general practice team to meet the needs of patients, supporting the
delivery of policy and procedures, and providing nurse leadership and
mentorship as required. The post holder will also contribute to the practices
delivery of the Quality and Outcomes Framework (QOF).
