Job summary
SLC Medical Group are looking for a highly motivated clinician to join our Frailty Team. We welcome applications from: Frailty Nurses, Nurse Practitioners and Paramedics.
The Frailty Team are responsible for assessment and co-ordination of care and services to people with multiple co-morbidities and complex needs who live in their own homes or care homes.
Working autonomously you will deliver safe, effective clinical decision-making and expert quality care, including assessment, diagnosis, clinical management and evaluation to the our frail population. You will have the attitude, skills and knowledge base to deliver quality holistic care and treatment.
You will assist in the provision of patient care at the surgery, or in the patients place of residence, working in collaboration with a multidisciplinary team.
You must work at all times within your own competence and adhere to CQC standards.
Main duties of the job
- Assessment of individual patient needs
- Initiate Investigations, and with support determine a differential diagnosis and initiate appropriate holistic, evidence based treatment and care.
- Co-ordination of care
- Refer patients who require medical intervention to appropriate services
- Work within a Multi Disciplinary Team
About us
We are a successful, high achieving, PMS training practice. We are located at three sites in Grimsby and Cleethorpes with a list size of 24,000 patients.
