A rare and exciting opportunity has arisen to appoint for an experienced, friendly, and enthusiastic Diabetes Specialist Nurse Nurse to join our highly regarded team at Greenway Community Practice.

We are seeking a Diabetes Specialist Nurse to join our team on a part time, permanent basis. As a specialist in the role, you will work closely with the Clinical Nurse Lead and the GP Diabetes Lead to provide specialist Diabetes clinics to our patient population.

You will be responsible for carrying out Patient diabetic reviews. The role focuses on maintaining clinical standards and improving services for patients with long term conditions.

This is an excellent opportunity for a local Practice Nurse who has a specialist interest in Diabetes care/management and is committed to providing high-quality care in a primary care setting.

Main duties of the job

Scope and Purpose of the role

  • To provide expert care, education, and support for people with diabetes, aiming to improve quality of life and self-management.
  • Undertake regular diabetes reviews for patients with Type 1, Type 2 and pre-diabetes in line with local and national guidelines
  • Monitor and assess glycaemic control, including HbA1c results, blood glucose records and trends
  • Provide lifestyle advice and structured education on diet, physical activity, smoking cessation and self-management.
  • Where trained, support initiation, titration and monitoring of diabetes medications, including oral agents and injectable therapies.
  • Maintain accurate, up-to-date clinical records and contribute to diabetes audits and QOF requirements.
  • Liaising with GPs and other healthcare professionals to ensure integrated care.
  • Assess and prioritise patients health needs, taking appropriate action to support their care and wellbeing.
  • Deliver high-quality patient care using advanced nursing knowledge and specialist clinical skills.
  • Deliver routine treatment room services in line with practice protocols.
  • Perform procedures such as wound care and dressings, Injections and immunisations, Venepuncture, blood pressure monitoring and removal of sutures and staples
  • Ensure infection prevention and control standards are adhered to at all times.
  • Work collaboratively within the multidisciplinary team to deliver high-quality patient care.

Greenway Community Practice is located in Southmead, a northern suburb and council ward of Bristol. Filton (in South Gloucestershire) and the Bristol suburbs of Monks Park, Horfield, Henleaze and Westbury on Trym are on its boundaries.

You would be part of a strong and experienced clinical team comprising of 5 GP partners, 5 salaried GP's, 2 clinical pharmacists, 4 Nurses and 4 HCA's a Paramedic Practitioner and a First Contact Physiotherapist.

Job responsibilities

Working in accordance with the NMC Code of Conduct, the Diabetes Practice Nurse will provide high-quality, patient-centred diabetes care alongside general treatment room nursing duties. The post holder will be responsible for undertaking structured diabetes reviews, supporting patients with long-term condition management, and delivering safe, effective treatment room services in accordance with practice protocols and national guidelines.

To practice unsupervised and independently in accordance with the NMC Code of Conduct and within individual competencies, with reference to Practice guidelines and referring on to other team members as appropriate

  • To perform routine tasks related to patient care, to include
  • Wound / leg ulcer care management (including doppler examination)
  • Undertaking cervical cytology
  • Ear irrigation
  • Injections and immunisations (including childhood immunisations)
  • Providing a comprehensive travel advice and vaccination service
  • Obtaining pathology samples, swabs, urine, blood etc.
  • Initiate and perform ECGs
  • New patient checks
  • Spirometry
  • To chaperone GPs and assist patients (where appropriate) during clinical procedures
  • To assist with minor surgical procedures as necessary
  • To provide general and specific health screening and health education to the practice population with referral to GPs as necessary

  • To advise patients on general health care and minor ailments
  • To order and maintain adequate supplies of stock (including vaccinations) and equipment relating to patient care

  • To advise Senior Nurse of potential problems or errors within the range of assigned tasks
  • To develop the care of patients with complex health needs, which may involve identification and assessment of such patients and the subsequent introduction and use of technical and specialist nursing treatments
  • Undertake structured diabetes reviews for patients with Type 1, Type 2 and pre-diabetes in line with NICE and local guidelines.
  • Monitor and assess HbA1c, blood glucose results and other relevant investigations.
  • Review cardiovascular risk factors including blood pressure, lipids, weight and BMI.

  • Identify patients requiring escalation of care and liaise with GPs, diabetes specialist nurses and secondary care teams as appropriate.
