Main duties of the job
You will be assisting trained nurses and other health care
professionals in the delivery of the highest standard of patient care, ensuring
that this is sensitive to the needs of the patient.
You will manage your own clinics, delivering
long term condition checks, dressings, blood tests, etc. both in practice and
where required in patients homes

Patient Care
- Undertake
routine diagnostic nursing tasks such as: ECGs, Blood Pressure checks, Blood
Glucose monitoring, routine heights, weights and urinalysis
- Provide
wound management for the patients under the guidance of registered nurses.
- Complete health assessments on newly registered
patients and document simple medical history.
- Use
patient contacts as opportunities to promote health education, including
education on practice systems such as obtaining a blood result.
- Undertake
NHS Health Checks, using the opportunity to promote health and well being
- Complete long term condition monitoring with appropriate
training
- Act at all
times with regard to the confidentiality and
sensitivity of issues to do with patients, their
care and treatment.
- Record and document all patient contact.
Governance
- Adhere to the nursing protocols in place, and contribute to Practice
audits
- Participate in programmes of continuing professional development
and training under the guidance of
the Lead Nurse
- Attend clinical and team meetings and tutorials as required
- Undertake mandatory and statutory training
