Job summary

Westrop Medical Practice is seeking an experienced Advanced Clinical Practitioner to join our clinical team and undertake autonomous clinics as part of the GP rota.

Westrop is a large, multi-site GP partnership serving approximately 49,000 patients across six sites in North Swindon. Our multidisciplinary team includes GPs, nurses, pharmacists, paramedics, physiotherapists and care navigators working together to manage both same-day demand and ongoing care for our patients.

This role is suited to a confident ACP who is comfortable managing undifferentiated presentations, making independent clinical decisions and contributing to the day-to-day management of acute and routine GP workload.

The successful candidate will run their own clinics within the GP rota, assessing, diagnosing and managing patients, arranging investigations, prescribing where appropriate and coordinating care with the wider multidisciplinary team.

You will be supported by an experienced clinical team and allocated a GP mentor, ensuring you have consistent clinical support, opportunities for discussion and ongoing professional development. While the role requires a high degree of autonomy, Westrop prides itself on maintaining a collaborative and supportive clinical culture where colleagues are always available to discuss complex cases.

Main duties of the job

The Advanced Clinical Practitioner will undertake autonomous clinical assessment and management of patients as part of the GP rota, working across Westrop Medical Practice sites as required.

The role will include assessing and managing patients presenting with undifferentiated and undiagnosed conditions, undertaking clinical examinations, formulating differential diagnoses and implementing appropriate management plans. This will include prescribing where appropriate, arranging and interpreting investigations, and making referrals to secondary care or community services.

The ACP will contribute to both same-day demand and routine GP workload, providing safe, effective and patient-centred care. The postholder will also support the wider multidisciplinary team through shared clinical discussions and collaborative working.

Accurate and timely clinical documentation, coding and completion of administrative tasks (including results management, prescriptions and correspondence) will form an essential part of the role.

The ACP will work with a high level of professional autonomy while having access to support from the GP team. The postholder will be allocated a GP mentor and will be expected to engage in regular clinical discussion, peer learning and continued professional development.

The role requires strong clinical judgement, excellent communication skills and a commitment to maintaining high standards of patient care within a busy primary care environment.
