Main duties of the job
The ideal candidate will be a qualified Practice Nurse with experience in a primary care setting.
Teamwork is at the heart of our culture, with regular MDT and clinical meetings supporting collaborative working across the practice. We are a welcoming, friendly and supportive team who work hard to deliver high-quality care and improve health outcomes for our patients.
The successful candidate will be expected to contribute to the delivery of the practices Quality and Outcomes Framework (QOF) programme and support the wider clinical team in maintaining high standards of patient care.
We are an active teaching practice with GP trainees, junior doctors and student nurses. The successful candidate will be supported by a GP mentor and will have access to regular teaching and development opportunities where required.
Key responsibilities will include:

Main Responsibilities

Provide assessment, screening and treatment services for patients

Deliver health education and preventative health advice

Manage treatment room supplies and equipment

Undertake pathological specimen collection and other investigatory procedures

Main Duties and ResponsibilitiesClinical Duties
The Practice Nurse will:

Assess patient needs and provide appropriate nursing care either independently or in conjunction with a GP or other healthcare professional

Deliver treatment room services including wound care, venepuncture, ECGs and other clinical procedures

Undertake immunisations and vaccinations

Assist with the management of patients with long-term conditions

Carry out screening programmes including cervical cytology

Provide health promotion and lifestyle advice to patients

Recognise and respond appropriately to common childhood illnesses

Evaluate the outcomes of care and adapt care plans accordingly
-
Maintain accurate and contemporaneous clinical records
Triage and Patient Assessment
The post holder will participate in protocol-led triage, including telephone and face-to-face assessment of patients, providing clinical advice or arranging appropriate follow-up with other healthcare professionals where required.
Treatment Room Responsibilities
