Job summary

We have an exciting opportunity for a Dietician to join our Primary Care Network. The role will be supporting our GP practices within Westrop Medical Practice (Brunel PCN 4).

The experienced Dietician will undertake clinical assessments of patients to learn about their health goals, dietary needs, preferences and restrictions. They will provide specialst nutrition and diet advice to patients and carers.

One of the key aims is supporting the practice with our innovative Metabolic Clinic which aims to help patients with metabolic syndrome (including Type 2 Diabetes) to undertake sustainable dietary changes to help improve their condition, without relying on medication. As part of the metabolic team you will work alongside our lead GP, and have a role in further development of this service, as well as helping to conduct patient reviews.

Other areas of focus of the role will be assisting patients with weight loss, IBS, annual Coeliac reviews and advice to frail patients who require support to gain weight.

This role will be working autonomously, but will have the support of a multi-professional primary care team made up of doctors, nurses, clinical pharmacists, physiotherapist, mental Health nurses and pharmacy technician.

Main duties of the job

The appropriate individual will be a qualified Dietician with BSc in Nutrition & Dietetics (or equivalent) with a registration with the British Dietetic Association.

You will have strong experience ofworking in Nutrition & Dietetics within a healthcare setting.

The successful candidate will be able to advise patients on special dietary requirements (i.e. food allergies, coeliac disease, diabetes etc.) as well as educating health professionals and the public about nutrition.

You will have strong experience and knowledge of working with different patient groups and needs across a diverse population. You will have excellent communication skills in addressing and providing health advice to patients to learn about their health goals and dietary needs, preferences, and restrictions.

You will be used to working within a multi-disciplinary team.

JOB DESCRIPTION:You will provide a dietetic service to Brunel 4 Primary Care Network including acting as First Contact Practitioner Dietitian with expert knowledge in diabetes and weight management.

You will provide specialist diabetes dietetic led clinics for people with diabetes where appropriate.

You will work as a highly autonomous practitioner, within a primary care multi-professional team demonstrating a broad knowledge across all areas of dietetics.

You will work to develop, through co-production, new pathways of care, referral criteria, local guidelines and protocols.

You will establish means of evaluating pathways of care and patient outcomes.

Leads on supporting referrals to local and national structured education programs that are commissioned within the locality.

Lead on the development of diabetes nutrition education for other members of the MDT in Primary Care.

Education and training of students and practice staff.

Continue to develop and expand upon own personal clinical expertise

To participate in training and development opportunities that accompany the role

To abide by the professional standards of practice as set out by HCPC

To ensure safe practice at all times through the recognition and appreciation of clinical governance

Liaison
