Job summary

Working under the direct supervision of the Nursing Manager and strictly in accordance with specific practice guidelines and protocols; the Healthcare Assistant will primarily assist the ANP Home Visit team in the support of housebound patients with long term conditions, supporting their reviews and ongoing care. This forms the core of the work stream, but the individual should ideally have the flexibility to be able to work within the practice setting if required.

The required working hours for this role will be 15-20 and will be discussed with the successful candidate.

Main duties of the job

  • Ensure the delivery of nursing care is evidence based and own practice is clinically reviewed through supervision and reflective practice as part of revalidation.
  • Provide clinical support to the Long Term Conditions team and promote collaborative working relationships between all members of the multi-disciplinary team.
  • Manage and control effective and economic use of diagnostic and screening equipment
  • Have knowledge and experience of the Long Term Condition reviews in either pts home or clinical settings
  • Manage administrative patient task and documents in a timely manner
  • Carry out home visits to house bound patients
  • ECG recording
  • Chaperoning duties
  • Process and management of laboratory samples requested by GPs/nurses
  • Sterilising, cleansing and maintenance of surgical equipment
  • Vaccine/cold chain storage, monitoring and recording
  • Vaccine stock management and rotation, to include fridge temperature recording
  • Prepare and maintain environments and equipment before, during and after patient care interventions including assisting GPs during the performance of minor operations

Health & Safety:

The post-holder will contribute towards the development of and adhere to Practice Health and Safety procedures, particularly with regard to Infection Control Procedures. This will include (but will not be limited to):

  • Using personal security systems within the workplace according to practice guidelines
  • Awareness of national standards of infection control and cleanliness and regulatory / contractual / professional requirements, and good practice guidelines
  • Responsibility for the correct and safe management of the specimens process including collection, labelling, handling, use of correct and clean containers, storage and transport arrangements
  • Management and maintenance of Personal Protective Equipment (PPE) for the practice including provision, ordering, availability and on-going correct usage by staff
  • Adherence to Practice hand hygiene policy.
  • Ownership of infection control and clinically based patient care protocols, and implementation of those protocols across the practice
  • Active observation of current working practices across the practice in relation to infection control, cleanliness and related activities, ensuring that procedures are followed and weaknesses / training needs are identified, escalating issues as appropriate
  • Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks across clinical and patient process
  • Making effective use of training to update knowledge and skills, and initiate and manage the training of others across the full range of infection control and patient processes
