Job summary

The successful candidate will responsibly undertake clinical medication reviews to proactively manage patients and chronic diseases. You shall work as part of a multi-disciplinary team and be the primary support to general practice staff regarding prescription and medication queries.

Main duties of the job

You will provide routine and specialist clinics within your working week including routine medication reviews, acute prescription requests, repeat prescriptions and urgent same day queries, whilst providing expertise in clinical medicines advice. The post holder will be resolute and motivated to ensure the integration of the practice with hospital and community pharmacies to improve the operational efficiencies and quality of care and service within the practice.

Job description

Job responsibilities

The post holder will:

Clinical Responsibilities

  • Clinical medication reviews
  • Review patients monitoring needs
  • Same day medication queries
  • Urgent medication requests
  • Provide medicines optimisation to patients
  • Provide clinics for patients with queries, concerns or questions on their medication
  • Provide telephone help lines for patients with queries, concerns or questions on their medication
  • Provide follow-ups for patients to monitor the effect of any medication changes
  • Signpost and refer to GPs or other healthcare professionals where appropriate
  • Review patients with single or multiple medical problems when medicine optimisation is required
  • Make appropriate recommendations to the clinical team for medicine improvement, prescribing and monitoring
  • Organise changes to reduce the risks of prescribing medicines to high risk patients
  • Respond appropriately to medicine-related enquiries from clinical staff, other healthcare teams, patients and other practice staff
  • Work with care home staff to improve the safety of medicines ordering and administration
  • Maintain confidentiality at all times
  • Adhere to the practice prescribing formulary, local and national guidelines

  • Effectively manage the nominated areas of responsibility as allocated

  • Develop harmonious and effective relationships across all practice teams and with patients

Administration Responsibilities

  • Ensure accurate and timely summarising of patient medical records using appropriate written and electronic documentation according to the practice guidelines and policies
  • Contribute and participate in audits, research, evaluation and clinical standard setting within the Practice related to prescribing areas, provide feedback and employ any changes
  • Work with all governance and internal systems relating to the management of clinical systems and data
  • Record clear IT based review notes to agreed standards
  • Ensure all practice policies are fully applied
  • Fully document all aspects of patient care and complete all required paperwork for legal and administrative purposes

Training and Responsibilities

  • Be actively involved in own development and training
  • Undertake effective professional development as necessary to keep up to date with current requirements and in line with Practice policy

  • Undertake any relevant training as required relating to organisational standards and changes to software or systems
  • Support and work closely with administrative and other clinical members of staff in the organisation/improvement of the practice systems for Clinical Governance and Quality Indicator targets
  • Demonstrate skills and activities to others who may be undertaking similar work
  • Attend courses (internal and external) required to ensure professional development and improvements are met with new guidelines
  • To keep up to date with latest developments/guidelines
