Job summary
A position has arisen for a part time Practice Nurse to complement our existing nursing team working 30 hours per week.
Experience in Practice Nursing essential including Chronic Disease Management.
Holiday entitlement will be 5 weeks + bank holidayspro rata .

Main duties of the job
- To provide support to the Practice Nurse Team, in collaboration with the General Practitioners, to develop and maintain an effective, quality driven Practice Nurse Service
- To participate in integrated nursing practice within the wider nursing team (Practice Nurses, Community Nurses and Health Visitors) underpinned by the development of evidence based practice for shared delivery of care to the whole practice population. To participate in the monitoring of key quality markers to ensure the delivery of clinically effective care. This includes QOF/IFF (and any subsequent scheme) and local and national enhanced services.

We are a well established forward thinking GMS practice comprising 5 GP partners, 1 Advanced Nurse Practitioner Partner, 1 Clinical Pharmacist, 2 Emergency Care Practitioners, 2 Practice Nurses, 1 HCAand a Phlebotomistsupported by an excellent reception/admin team.

RESPONSIBILITIES
- The Practice Nurse is employed by the GP's and is a member of the team responsible for the clinical nursing care of the practice population together with the community nursing team.
- It is the Practice Nurses responsibility to maintain professional registration with the Nursing and Midwifery Council (NMC) and to undergo revalidation as per standard requirements.
- The Practice Nurse will engage with annual appraisal within the Practice and ensure that she keeps up to date with current practice and guidance and clinical and mandatory training requirements.
DUTIES

- Practice independently in accordance with the NMC code of conduct and with reference to practice, local and national guidelines, appropriately referring to GP's where appropriate.
- Arrangements for and attendance at medical sessions to carry out immunisations (including children) and screening programmes.
- Clinics to include Chronic Disease Management including - Asthma / COPD / Diabetes.
- Smears
- Clinics may also include Venepuncture, Injections and wound management. This list is not exhaustive.
- Assisting and / or chaperoning the GP's during clinical procedures and medical sessions, and assisting with Minor Surgery
- Providing nursing treatments under the medical direction of the Partners and PGD's
- Liaising closely with the GP's and the Community Nursing Team and MDT concerning the care and follow up of patients.
- Advising patients and health staff on health matters
- Teaching patients and their carer (where appropriate) how to self-care
- To maintain fridges at recommended temperatures and ensure accurate recording of fridge temperatures. To check emergency drug and anaphylaxis kit supplies, maintain accurate records and ensure timely supply.
- Maintaining accurate and relevant nursing records using computerised clinical system and written records as appropriate
- Liaising with all members of the wider primary care team
- Participation in the training programme of nursing students and students from other professions as appropriate.
- Participation in relevant clinical / nursing audits
- Other duties consistent with the nurses qualifications as determined by the general practitioners as require.
- Ensuring standards are maintained within the practice in line with practice protocols and CQC guidance.
- Attendance at Clinical Team Meetings and Nurse Team Meetings monthly and ad hoc should they be required.
