Job summary

Pioneer Medical Group comprises of 3 practices including Avonmouth Medical Centre, Ridingleaze Health Hub and Bradgate Surgery. We are seeking friendly, kind, professional and enthusiastic Care Navigators to help us manage and deliver first class healthcare to our patients.

Main duties of the job

Most of your role will be telephone based, and duties will typically include:

- Assisting and signposting patients in accessing the appropriate service or healthcare professional

- Providing general assistance to the practice team and other healthcare providers

- Providing non-clinical information or advice to patients / relatives

- Work alongside the duty team to ensure patients receive the right care at the right time from the right person

- Ensuring that appointments are booked as per practice protocols

Other tasks include: registering patients, maintaining patient records, managing patients medication requests, scanning documents and reception duties. You will also be required to undertake any other duties that may be reasonably requested of you to help the department run smoothly.
