Care Coordinator

Job summary

The focus of the Care Coordinator role is to create opportunities to improve the patient experience and ensure patients are directed to the best source of help for their needs and circumstances in a timely manner.

This post requires communication with multiple agencies outside of Seaford Medical Practice and also multi-disciplinary working within the practice. Also within the scope of this role is a contribution towards the quality of medical records. Together, all aspects of this role require a sound understanding of medical terminology and of the patient experience. Our ideal candidate may have:

• Ability to work autonomously in a highly pressured environment.
• Experience of or a good working knowledge of how General Practice works.
• An understanding of equality and diversity and enthusiasm for improving the care of marginalised groups.
• Excellent inter-personal skills.

About us

Seaford Medical Practice has a clinical team consisting of 6 GP Partners, 5 salaried GPs, 3 Sisters, 5 Practice Nurses, 6 HCAs, 1 Phlebotomist, 2 Paramedics, 3 ACP's, Clinical Pharmacist, First Contact Practitioner Team. The patient list is currently 18k patients.

Seaford Medical Practice is a training practice, and hosts GP Speciality Trainees as well as FY2s, Nursing students from Brighton University, and Medical students from BSMS.

The practice is committed to ensure we develop and train the future workforce for Primary Care.
