2 x Care Coordinator roles
Working Hours 8am to 1pm (Mon to Fri) = 25 hours per week
We are looking to recruit a Care Coordinator who will work as part of the multi-disciplinary team in Windsor House Group Practice, building on our successful and existing model.
The successful candidate will be able to work closely with practice management and the practice support teams in coordinating all key activities including access to services, advice, and information, and ensuring health and care planning is timely, efficient, and patient-centred via the telephone.
Windsor House Group Practice is a large GP Practice with 3 x surgeries based in Leeds (Morley, Churwell and Drighlington) with a list size of over 18,900 patients.
The successful candidate is expected to provide an efficient admin support service to patients and staff.
To provide an efficient initial, advice and guidance service for
patients of the practice making enquiries around the services provided.
Main duties of the job
The successful candidate should have previous experience working in an administrative environment with excellent IT skills,the ability to work with people at all levels, together with good organisational and communication skills, both verbal and written. You should have the ability to use your own initiative but also be team orientated and have a flexible approach to work.
You will need to have good time management skills, the ability to organise and prioritise work along with an understanding of, and commitment to, patient confidentiality.
A willingness to learn and develop skills will be positively encouraged. You will be part of an enthusiastic team, who will play an instrumental part in the smooth running of the practice.
About us
Windsor House Group Practice is a large GP Practice in Leeds spanning across three sites. We have a team of approx 65 employees both clinical and administrative. We are passionate around the care and support of our patients and the role of a receptionist is key to our service provision.
The role will primarily work at Adwalton House Surgery, Drighlington. (with an expectation that the successful candidate would work at one of the other surgeries as and when required).
The working hours for the two roles are 8am to 1pm (Mon to Fri)

- To plan and organise own work schedule with the use of appropriate planning aids, demonstrating good time management and ensure anticipated difficulties in meeting deadlines are promptly reported.
- Deal with patient, management and clinical requests for actioning urgent correspondence, reports and any other appropriate adhoc correspondence requested in line with the practices policies and procedures
- To obtain and organise information in support of own work activities, maintain confidentiality in accordance with organisation procedures.
- Answer queries from colleagues, GP's and patients in a timely and professional manner.
- To ensure that secure systems are in place for the storage of all resources including computerised information.
- Scanning and uploading relevant patient medical documents into the practices Electronic Patient Record system.
- To ensure all phone calls are dealt with in a professional and timely manner.
- To operate the various IT systems used to fulfil the role/department requirements.
