Job summary

We are seeking a proactive and skilled Pharmacy Technician to join our dedicated team working solely within Leckhampton Surgery. This pivotal role acts as a vital link between doctors, nurses, and other clinicians, bridging the gap between clinical care and operational excellence.

As part of our multidisciplinary team, you will support the delivery of high-quality patient care by contributing to medicines optimisation, drug monitoring, and long-term condition management. You will play a key role in ensuring compliance with CQC standards, supporting QOF requirements, and enhancing the safety and effectiveness of prescribing practices.

Main duties of the job

Working collaboratively with our clinical and administrative teams, you will play a key role in embedding best practices in pharmaceutical care, enhancing workflow efficiency, and improving patient outcomes. This is a patient-facing role, based within the surgery, offering a unique opportunity to make a meaningful contribution at the core of primary care.

As a Pharmacy Technician, you will support the safe, effective, and efficient management of our patient caseload through robust systems for repeat prescribing, medicines optimisation, waste reduction, and maximising therapeutic outcomes. Key responsibilities include:

Responding to medicines-related queries and requests from patients and clinicians

Conducting medicines reconciliation following hospital discharge, transitions into care homes, or intermediate care settingsidentifying and resolving discrepancies, and liaising with patients, care coordinators, and community pharmacists

Reviewing and reconciling medications following specialist or secondary care input

Collaborating with clinicians to develop and implement systems for shared care prescribing, high-risk drug monitoring, and optimisation of medicines for long-term conditions

Delivering travel health advice and immunisations under Patient Group Directions (PGDs)

Running your own clinics (following appropriate training and competency assessment), including hypertension reviews, respiratory condition reviews, and womens health consultations

JOB SUMMARY

To support the clinical team in delivering high-quality medicines optimisation and patient care. The Pharmacy Technician will undertake both patient-facing and administrative roles, contributing to safe prescribing, medicines reconciliation, and public health initiatives. The role includes supporting audits, clinical reviews, and the implementation of national prescribing policies.

Clinical Responsibilities

Undertake patient-facing and patient-supporting roles to promote effective medicines use through shared decision-making.

Conduct medicines optimisation tasks including checking inhaler technique, supporting medication reviews, and medicines reconciliation.

Support medication reviews and medicines reconciliation for new care home patients and during patient transfers between care settings.

Provide lifestyle advice and service information to address public health and social care needs.

Maintain a central role in shared care protocols and liaise with pharmacists or GP for complex cases.

Assist in the delivery of medicines optimisation incentive schemes and patient safety audits such as antimicrobial stewardship initiatives

Support implementation of national prescribing policies and guidance through clinical audits and quality improvement measures.

Contribute to public health campaigns (e.g., COVID-19 or flu clinics) through advice or direct care.

Implement vaccination programmes for adults and children.

Develop, implement and embed health promotion and wellbeing programmes.

Implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing.

Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health.

Technical and Administrative Responsibilities

Work with the multi-disciplinary team to implement efficient medicines ordering, returns, and waste reduction processes.

Supervise reception teams in sorting and streaming prescription requests to ensure clinical staff focus on complex cases.

Lead medicines optimisation systems including electronic prescriptions, safe repeat prescribing, and high-risk medicines monitoring.

Provide training and support on the legal, safe, and secure handling of medicines, including EPS implementation.

Develop relationships with pharmacy colleagues across primary care, community pharmacy, secondary care, and mental health services.

Support delivery of QOF, QIPP, incentive schemes, and other quality or cost-effectiveness initiatives.

Undertake tasks consistent with the level of the post and scope of the role, ensuring timely and effective delivery.

Direct Patient- Care Responsibilities

Obtain patient consent for the collection of blood samples.

Perform venepuncture in accordance with practice protocols and infection control standards.

Maintain accurate patient records on the clinical IT system following sample collection.

Ensure correct labelling, packaging, and storage of samples prior to dispatch.

Ensure timely dispatch of samples to the NHS Trust laboratory.

Work collaboratively with the nursing team, seeking guidance when necessary for patient-related matters.

Coordinate the provision of travel medicine services.

Conduct basic patient observations such as blood pressure, pulse, temperature, and respiration rate.
