Job summary
The Young Person Link Worker is a vital member of the Berkeley Vale PCN team, acting as a bridge between primary care, young people, their families, and community services. This role supports young people (typically aged between 11 and 25) who present with social, emotional, and mental health concerns by providing early intervention, assessment, personalized support planning, and onward referrals.
Main duties of the job
This position goes beyond traditional "signposting," involving mental health triage, short-term low-level support, navigation of neurodiversity pathways, and liaison with schools and external agencies. The Link Worker also plays a key role in managing referrals for local youth projects and maintaining clear communication channels between GP practices, schools, parents, and statutory services.

This is an exciting opportunity to join the Berkeley Vale Primary Care Networks team of dedicated professionals. The role is based at May Lane Surgery and covers patients at both May Lane and Cam & Uley Surgeries. Both sites are located near the market town of Dursley, Gloucestershire.
We are looking for an enthusiastic, empathetic, and well-organised individual to join our team.
Ideally, we are seeking someone who can work 30 hours per week across 4 days.
Interviews are expected to take place on 6th November 2025.
