Job responsibilities

Mental Health Triage & Support

Receive referrals from GPs and receptionists via clinical tasks or email.

Conduct triage via telephone or face-to-face assessments with young people and/or their parents.

Signpost or refer to external services (e.g., CAMHS, counselling, mentoring, Early Help).

Provide follow-up calls (4-12 weeks later) to ensure referrals have been actioned and additional needs addressed.

Offer short-term, low-level 1:1 support where appropriate (e.g., weekly sessions with six-week reviews).

Liaise with schools, Early Help teams, and other professionals to ensure a joined-up approach to care.

Escalate safeguarding concerns in line with PCN and practice policies.

2. Neurodiversity Pathway Support

Manage referrals related to suspected ADHD/ASD, including guidance on Gloucestershire NHS pathways and Right to Choose options.

Signpost and complete referral paperwork and administration.

Liaise with parents, carers, and schools to track referral progress.

Support families in navigating long wait times and encourage collaboration with school SENCOs.

Maintain up-to-date knowledge of national and local neurodiversity resources and share this with practice staff.

Social Prescribing & Community Connection

Collaborate with young people to create personalised care plans addressing practical, social, and emotional needs.

Connect young people to local groups, mentoring schemes, counselling, youth PPG group and other non-medical support services.

Provide advocacy when needed, such as attending appointments or assisting with service applications.

Collaboration & Process Development

Work closely with GPs, receptionists, and admin staff to develop robust and consistent referral processes.

Share information on common themes to support PCN planning.

Help shape future service delivery by identifying gaps and suggesting improvements.

Record Keeping & Reporting

Maintain a secure caseload and accurate clinical records.

Regularly update the practice team on patient progress and outstanding actions.

Contribute data for PCN reporting and quality improvement initiatives.

Person Specification

Qualifications

Essential

• Evidence of a sound level of education.

Desirable

• Qualifications based around working with children and young people.

Experience

Essential

• Experience working with children and young people in health, education, or community settings.
• Strong triage, assessment, and case management skills.
• Knowledge of local and national mental health and neurodiversity pathways, or the ability to learn quickly.
• Excellent communication and listening skills, with the ability to build trust rapidly.
• Ability to manage a high-volume caseload and prioritise effectively.
• Skilled in multi-agency working, with experience liaising with schools, parents, and statutory services.
• Competent in IT systems and secure record keeping.
• Commitment to safeguarding, confidentiality, and equality, diversity & inclusion.

Desirable

• Experience delivering low-level therapeutic interventions (e.g., CBT-based approaches, mentoring).
• Mental Health First Aid or equivalent training.
• Knowledge of Right to Choose and local referral pathways for ADHD/ASD assessments.
