May Lane Surgery is a caring and friendly GP
Practice dedicated to delivering excellent healthcare to our patients. We are
looking for an experienced Health Care Assistant to join our team. If you are
enthusiastic and have great people and communication skills, we would like to
hear from you.
The role is a split role within the surgery consisting of HCA clinics and being part of our Frailty Team which will including calling patients and visiting them in their homes.
Main duties of the job
The HCA role involves a variety of duties including
Phlebotomy (venepuncture), ECG's, BPs, and urinalysis, long-term condition checks, Health checks and wound checks. (This
list is not exhaustive and may change to the needs of the nursing team).
The fundamental part of the HCA Frailty role is to meet the primary care needs of an identified group of people
identified as frail and needing further support in the community.You would be expected to undertake home visits in accordance with the relevant protocols, support patients and families to help patients safely stay at home.

We are a 3 partner Surgery with
a comprehensive team including reception, nursing, frailty and managerial staff. We have an
enthusiastic ARR team currently comprising of Clinical Pharmacists, Pharmacy
Technician, Paramedic, Social Prescriber and a Young
Persons Social Prescriber.

To undertake and record the following tasks (not exhaustive)
- Height
- weight (BMI)
- Blood pressure
- Pulse
- Temperature
- Venepuncture
- ECG'S
- Urinalysis
- Diabetic Foot Checks
- NHS health checks
- Smoking cessation
- Administer B12 injections under PSD
- Chronic disease management as applicable
- Assist with Minor Ops
- Recognise changes and abnormalities in patients conditions and act accordingly - notifying GP/Nurse of any significant issues
- Stock control and ordering
- Fridge temperature recording
- Legionella/Water temp checks and recording
- Awareness of national standards of infection control and cleanliness and regulatory/ contractual/ professional requirements and good practice guidelines.
- Act as chaperone
- Maintain confidentiality
- Processing of patient samples
- Safe management of sharps procedures, including training , use, storage and disposal
- Waste management
- Spillage control procedures
Knowledge of Information Governance and Caldicott Principles
To undertake home visits in accordance with the relevant protocols
Support patients and their families/care home staff to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care
Completing and sharing advanced care plans
To be responsible for patient safety through knowledge of systems, legal requirements and understanding of litigation.
Provision of support to carers considering what can put in place to support that persons mental health and wellbeing, by the use of tools to identify deterioration in wellbeing and mental state.
Liaison between local organisations to provide services that can support the delivery of care within the patients own home.
Referral to and attendance at MDTs involving a range of health and care professionals, from one or more organisations, working together to deliver comprehensive patient care.
