Job summary
The candidate will work in a large, busy, modern Meddygfa Glan Cynon Surgery with a list size of10,000 patients. The post holder will join a team of Practice Nurses,Three Health Care Support Workers and two phlebotomists providing clinical support in diabetes, respiratory, women's health, family planning, wounds and more. We are looking for an experienced practice nurse to take the lead in chronic disease management
Main duties of the job
Duties / Responsibilities
Chronic disease management including diabetes, respiratory and womens health
Wound management
Immunisations
Womens Health and family planning
Adherence to Infection Control procedures
Taking & recording observations such as BP monitoring
