Job summary
We are looking to welcome an additional Practice Nurse to join our friendly 3 partner practice who strive to always deliver excellent patient care across the whole practice. We are a supportive GP training practice committed to continued professional development across the whole practice.
The successful candidate will join an established team of salaried GP's, Advanced Nurse Practitioners, Health Care Assistants, Practice Nurses and Clinical Pharmacist and will be supported by the admin team, led by the Practice Manager.
We are looking for an enthusiastic and hardworking Practice
Nurse, with preferable chronic disease experience in Diabetes, Hypertension, Asthma and COPD.
The role is open to anyone with a registered nursing qualification, and whilst previous experience in general practice would be an advantage, we would still like to hear from you even if you do not have these skills, as training and development can be provided.
Pay dependant on experience.
Main duties of the job
You will be responsible for the delivery of practice nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population. The practice nurse will be responsible for a number of clinical areas such as health promotion, chronic disease management, health prevention, well women and well man clinics, as well as supporting the management team in the reviewing of clinical policy and procedure.
Duties to include:
- Family planning & women's health including cervical smears
- Chronic disease management, respiratory and diabetes
- Child immunisations and advice.
- Wound management.
- Travel health advice and vaccinations.
- Screening and helping patients to manage long term conditions.
- Administering injections.
- Giving advice, education and information about health conditions.
- Liaising with other health care professionals.
- writing records and keeping patients electronic notes up to date.

i. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary / specialist care as required
j. Maintain accurate clinical records in conjunction with extant legislation
k. Ensure read codes are used effectively
l. Maintain chronic disease registers

n. Manage patients on warfarin through INR clinics
