Job summary

An opportunity has arisen for a second Pharmacist to join our supportive team here at Glan Cynon Surgery to contribute to clinical work related to medicines. As part of the GP practice team you will help to improve value and outcomes from medicines and consult with and treat patients directly which includes providing extra help to manage long-term conditions, advice for those on multiple medicines and contribute to improving patient care.

Main duties of the job

  • Work with doctors and patients to address medicine adherence, reducing the wastage and overuse of medicines
  • Monitor patients blood results, focusing on potential harm that can come from long-term use of medicines or poor compliance
  • Review patients on complex medicine regimens/ with ongoing health problems
  • Support doctors by highlighting changes in NICE guidelines
  • Take part in multi-disciplinary case reviews
  • Deal with medication for patients recently discharged from hospital, including liaison with hospital, community and primary care colleagues
  • Work with the practice team to deliver repeat prescription reviews.
  • Look at how best practice is integrated into care processes and improve the quality of patient care, ensuring patient safety
  • Manage and prescribe* for long term condition patients

Job description

Job responsibilities

Undertake clinical medication reviews for patients with single or multiple long term conditions where medicines optimisation is required.

To reconcile medicines following discharge from hospitals or intermediate care and working with patients and community pharmacists to ensure patients receive the medicines they need post-discharge and to reduce risk of readmission.

To be involved in multidisciplinary clinical meetings

To manage repeat prescription requests, resolving queries where possible within scope of practice. To ensure patients have appropriate monitoring tests in place when required

Provide independent information to patients with regard to medicines and prescribing changes, initiating further support from other healthcare professionals where appropriate

Participate in review and setting of policies relating to medicines management and prescribing and help ensure practices prescribe in accordance with local guidelines and formulary

To identify areas of clinical risk at network level and make recommendations to support the introduction of new working practices that will optimise the quality of prescribing and make more efficient use of network resources

To maintain own clinical and professional competence and be responsible for own continuous professional development

Work with practices to ensure full compliance of standards for safe and effective care

Qualifications

Essential

  • Completion of an undergraduate degree in Pharmacy, and registration with the General Pharmaceutical Council.
  • Minimum of 2 years post-graduate experience in pharmacy.
  • Member of the Royal Pharmaceutical Society (RPS).
  • Flexibility, enthusiasm and good clinical knowledge.
  • Have experience and an awareness of common acute and long-term conditions that are likely to be seen in general practice.

Desirable

  • Experience of working in Primary Care.
  • IT including spreadsheet and database packages.
