Job summary

We are seeking to recruit a Lead Prescription Clerk to join our team to ensure patients receive a high quality of care regarding their medication process. You will lead our established team of prescription clerks and motivate them to do their job efficiently and arrange any ongoing training as appropriate.

You will need excellent communication skills as you will be communicating with external Pharmacies and health Care Professionals. Accurate data entry skills are compulsory for this role.

Main duties of the job

Answering medication queries from patients, staff members and pharmacies/care homes, inputting prescription orders, to liaise with patients and healthcare professionals to obtain correct prescription information.

Collate information from medication related queries and forward to an appropriate healthcare professional as required. Comply with legal and professional requirements and in line with practice repeat prescription policy.

Be aware of monitoring requirements for selected drugs and follow up required monitoring with patients. Ensure appropriate health checks and medications reviews are undertaken.

Manage document workflow in relation to prescriptions, check and action emails. Establish and maintain filing and administrative systems so that written or computerised information is easily accessible and secure.

Ensure there is appropriate cover and manage holiday leave requests for other members of the prescription team.

Undertake training as required to support safe and effective services for the Practice.

Ensure hospital discharges are processed and actioned as necessary.

Escalate any problems to Practice Manager

About us

We are a new build surgery serving Mountain Ash and surrounding areas with a list size of approximately 10,800 patients.

We have a motivated and friendly team consisting of 3 GP partners, supported by salaried GP's, Advanced Nurse Practitioners, Pharmacist, Nursing and Administrative team.

We are a training practice and encourage and support development of our practice staff.

Job description

Job responsibilities

Answering medication queries from patients, staff members and pharmacies/care homes, inputting prescription orders, ensure safe and appropriate storage of medicines

Collate information from medication related queries and forward to an appropriate healthcare professional in a timely manner

Allocate medication reviews ensuring patients have attended for chronic diseases reviews where appropriate

Liaise with patients and healthcare providers to obtain correct prescription information

Work with Prescribers to provide a responsive repeat prescription service, in accordance with any relevant and appropriate policy or protocol and template

Delivering the technical aspects of prescription preparation to ensure the safe prescribing of medicines

Comply with legal and professional requirements, accepted guidance on professional practice, and with safe systems of work for re-authorising repeat prescriptions

Liaise with pharmacies on the supply of prescribed drugs, alert clinicians of any supply issues and forward information on the recommended alternative

Be aware of monitoring requirements for selected drugs or categories of drugs and follow up required monitoring with patients concerned

Bring any queries to the attention of the relevant Prescriber to include under and over usage

Liaise with external pharmacy services and patients to resolve queries and communicate changes to medications

Build good working relationships with local Pharmacies to help enable the Practice prescription service

Check emails and tasks regularly, actioning them appropriately

Create tasks and assist clinicians in managing document workflow relating to prescriptions
