Clinical Pharmacist - GP Practice (12 Month Maternity Cover Post)
26.25 Hours per Week, working Monday, Wednesday & Friday
We are looking to appoint an enthusiastic, highly motivated clinical pharmacist to
join our clinical team. The successful candidate will be based in
general practice and will participate in the delivery of general practice
clinical pharmacy. This post is to cover for maternity leave on a fixed term 12 month contract.
The
role will involve undertaking medication reviews for the practice patients, health promotion, reviewing CVD risk, dealing with hospital discharge letters re medication changes and reviewing patient medication related queries.
Salary (£25 - £26 per hour) will be dependent on qualifications and experience with the
opportunity to join the NHS Pension Scheme
The successful candidate will have 2 years post qualification experience. A Masters degree in pharmacy (MPharm) or
equivalent and professional registration with the GPhC
Main duties of the job
The
primary responsibilities of the post holder will be:
Provision of direct patient care in the practice focussing on:
Medication Reviews with patients via clinics in the practice either face to face or by telephone, including reduction
of inappropriate poly-pharmacy and wasteful prescribing.
Management of medicines on transfer from secondary care
Hospital admission avoidance through medicine related incidence
Long Term Conditions management where medicines are the main
interventions
Development and oversight of systems and processes within the
practice to optimise the quality and safety of prescribing.
Administration duties to include: processing hospital discharging, patient queries, medication review waiting list work
An ability to work independently
and autonomously is essential. Support will be provided by the GPs.

JOB SUMMARY
Responsible for support to the partners in implementing effective medicine management and prescribing within the practice, identifying areas for improvement, and initiating and managing change of medications/ prescribing.
JOB RESPONSIBILITIES
The job holder must have an ability to fulfil and facilitate the below, however the degree of which tasks will be undertaken will depend on individual practice needs.
To develop and manage a medicines management plan and the delivery of patient services as determined by practice policy and local and national guidance.
Develop and manage medicines management, including delivery of training, in order to maximise cost-effective prescribing and to improve the quality of patient care.
Prescribing and writing scripts where appropriate and reviewing of hospital discharge medications
Patient services
To provide medication review services to patients via clinics in the practice, domiciliary visits and in residential and nursing homes, and to deliver pharmaceutical care plans that maximise cost-effective prescribing and improve the quality of patient care. Reduce inappropriate poly-pharmacy and wasteful prescribing through clinical medication review.
Improve and develop current practice systems.
Undertake Clinical Audits.
Actively work on relevant quality outcome framework targets.
Reconcile medicines following hospital discharge and work with patients and community team.
Contribute to reductions in medicine related hospital admissions and readmissions by supporting patients to get the best outcomes from their medicines and identifying and addressing medicines related issues.
Run Long Term Conditions clinics where medicines are the main interventions eg. Asthma
To present at patient group meetings or other appropriate events to give advice on the appropriate use of medicines.
To produce patient information leaflets and posters and run medicine awareness projects throughout the year.
Assist Partners with the appropriate monitoring and management of their prescribing budgets.
To prepare evidence based resources and information to support the medicine management team and all other relevant health professionals in the implementation of rational cost-effective prescribing.
To help plan, develop and support the introduction of new working processes within the practice to optimise the quality of prescribing.
Any other duty that reasonably corresponds with the role.

To produce bi monthly reports on medication reviews changes/ progress
To update and maintain accurate patient medication records on the practices clinical computer systems including advice given and action taken.

Implement drug withdrawals and alerts eg. MHRA aimed at improving medicines safety
Increase quality and safety of prescribing through mechanisms such as audit
Deal with hospital discharge reports and complete actions as directed in the report such as medicine changes
Deal with ad hoc patient medicine related queries
