Full Time Nurse Practitioner Role available

(Monday to Friday (Working Days Negotiable))

We are looking to appoint an enthusiastic, highly motivated nurse practitioner to join our nursing team. The successful candidate will ideally have experience working as a Nurse Practitioner.

The successful candidate will provide safe and effective health assessments, diagnose and treat patients at the practice and in the patient home. The role will involve responding to and treating patients with undifferentiated diagnosis. You will work as part of the multi-disciplinary team in providing quality; evidence based health care to meet the immediate needs of the patient.

Salary - £47-£52,000 WTE subject to negotiation, dependant on experience and extra skills with the opportunity to join the NHS Pension Scheme

Main duties of the job

The post holder will be an RGN with current NMC registration. They will also require a degree level clinical qualification. An ability to work independently and autonomously is essential. A Nurse Prescriber qualification and experience with telephone triage is preferable but not essential. There will also be opportunities for daily clinical discussions.

The practice will support the successful candidate in developing their skills in areas such as prescribing as required.

RESPONSIBLE TO: Dr Rebecca Clark

MAIN PURPOSE OF THE POST:

To provide nurse practitioner services to the patients of Winterton Medical Practice.

HEALTH AND SAFETY

Ensuring safe working practice.

Assist with implementation of Health and Safety requirements in the practice with the practice manager.

TEACHING AND MENTORSHIP

To take part in training when required and offer teaching and training support to other practice members when required.

Participate in annual appraisal.

Participate in training practice Registrar and medical students.

CLINICAL GOVERNANCE

Attend clinical meetings.

Participate in critical incident reporting and review and disseminate information or instigate changes as required.

Support practice members with audit as appropriate

Actively participate in quality monitoring relating to GP contract.

CLINICAL

Nurse Practitioner Acute and Routine Work

Offer same day appointments to patients with undifferentiated undiagnosed problems, assess, diagnose and treat including minor illness/injury.

Respond to and treat acute urgent conditions in the patient home

Triage patients by telephone that are requesting a same day appointment and offer advice, arrange treatment or book the patient an appointment within an agreed timeframe with the most appropriate clinician.

Face to Face, Telephone & Video Appointments

Organise and interpret appropriate investigations.

Onward referrals as appropriate

Referral to/liaison with GPs or other Healthcare professionals as appropriate.

Prescribe appropriate medication if required.

Management of patients with chronic disease.

Arrange appropriate follow up appointments

Ensure clinical practice is safe and effective and remains within boundaries of competence, and to acknowledge limitations.

Medication Reviews

Undertake medication reviews for individuals on any type of medication

Ensure quality framework monitoring is carried out as part of the medication review.

Amend doses/commence appropriate medications within own competency.

Refer to GP or other agencies as required.

Deal with patient queries in regard to their medication

Research

Participate in Good Clinical Practice training and training relevant to the studies undertaken by the practice.

Participate in studies as appropriate relevant to your role.

ADMINISTRATION

Deal with emails/tasks and other queries as required on a daily basis

Review blood results and take necessary action as appropriate

Ensure accurate and legible notes of all consultations and treatments are recorded in the patients electronic notes

GENERAL

Develop clinical skills to offer any additional services as agreed in the future.

Provide flexibility e.g. practice nurse cover/extra acute minor illness surgeries when required.

Participate in Nurse Team Meetings as necessary

Maintain appropriate registration with health service body

Ensure appropriate medical insurance indemnity cover is in place appropriate to your role (this will be funded by the practice)
