PCN Urgent Hub Manager

Job summary

St Helens North Primary Care Network are looking to recruit to a motivated and dynamic PCN Urgent Hub Manager on a fixed term basis to help manage and organise an Urgent Hub which has been created to help reduce pressure on GP practices. This role will involve working across all 6 practices in the North Primary Care Network.

We are looking for someone who has experience in Primary Care, is used to working on EMIS, enjoys patient interaction and is capable of working autonomously with minimal supervision.

The successful applicant will work within our friendly Primary Care Network multidisciplinary healthcare team and working closely with GP's who also work in the hub. They will also work alongside the wider ARRS team, and in partnership with the six practices that make up the Primary Care Network. This is a temporary role to cover maternity leave and we are looking to recruit someone to cover from February 26 to January 27.

Main duties of the job

Within our PCN, we have recently created an Urgent Hub model where all six PCN practices are able to book in to an acute hub clinic for same-day patients.

This is a full time role where the successful candidate will be expected to be based in practice alongside the Urgent Hub. This will involve working on a rota across multiple practices throughout the week. You will be responsible for adding clinics to EMIS, booking patients in as the arrive for the hub and any other reception or PCN duties as and when required. The hub runs from 10am to 6pm, and the successful candidate can work their hours from 8am to 6pm.

In this role, excellent communication skills will be required as there is interaction with patients. You will be expected to build good relationships with both ARRS and Practice staff and ensure the hub runs smoothly at all times. There will also be an expectation to help with administrative tasks within the PCN.

The successful candidate will be expected to manage the administration of the Urgent Care Hub and provide admin support to the Network Manager. They will liaise with Clinical Director and the six Practice Managers within the Network on a regular basis.

The postholder will have excellent communication skills, will be well organised with a proactive approach and have excellent knowledge of EMIS.

Principle Responsibilities

1. To fully manage the urgent care hub including:

-EMIS clinics

- staffing

- reception duties for the Hub in practices working across the Network surgeries.

- produce reports and provide data for monitoring purposes

- work with the PCN GP, ANPs, Network Manager, CD and Clinical Lead to make service improvements when required.

2. Liaising with Practice Managers in the Network to collate or disseminate information, including holding teams calls.

3. Liaising with ARRS staff in the Network to collate or disseminate information.

4. To provide administrative support in the procurement of equipment, uniforms, supplies and organising training sessions when needed.

5. Assisting Network Manager when required with the PCN Patient Participation Groups (PPG) for the Network to ensure patients aware of Network activities and feedback sought on services when appropriate.

6. Arranging Network meetings, collating the agenda, distributing information and typing minutes where required.

7. Supporting with the recruitment process for ARRS Staff.

8. Complete any training as required for the role.
