Job summary

A great opportunity to join our supportive and caring General Practice team!

We are a friendly and highly supportive training practice (8000 patients) in central London, close to transport links (Baker Street, Marylebone and Edgware Road stations).

We are looking for a committed, experienced Advanced Nurse Practitioner to help us provide high quality chronic disease management and manage acute, minor illness.

We train nurses so an interest in education would be a bonus.

This is a 1-year fixed contract position with option to extend.

Main duties of the job

As an Advanced Nurse Practitioner within the practice, you will:

  • Provide independent assessment, diagnosis, and management of patients with long-term conditions

  • Manage acute and urgent presentations

  • Prescribe and make clinical decisions within your scope of practice

  • Work closely with GPs and the wider multidisciplinary team

  • Undertake home visits whererequired

  • Maintain high-quality clinical records and contribute to service development and quality improvement

  • 1 Advanced Nurse Practitioner

  • Nursing team - Nurse associate and 2 HCAs

  • Pharmacy team

  • Administrative and reception teams

  • District nursing team on site

Job responsibilities

Undertake comprehensive patient assessments, including history taking, physical examination, diagnosis, and management of acute, chronic, and complex conditions;

Independently manage own caseload, including same-day urgent appointments;

Prescribe medications independently (where qualified) in line with practice policy and national guidance;

Request, interpret, and act upon diagnostic investigations (e.g. blood tests, ECGs, imaging);

Make timely referrals to secondary care, community services, and multidisciplinary teams;

Manage long-term conditions (e.g. diabetes, asthma, COPD, hypertension);

Provide health promotion, disease prevention, and patient education;

Work collaboratively with GPs, nurses and pharmacists;

Contribute to service development and quality improvement initiatives;

Maintain advanced clinical competence and professional registration;

Participate in clinical supervision, appraisal, and revalidation;

Support the learning and development of students and junior staff as appropriate;

Engage in continuing professional development (CPD) in line with NMC and practice requirements;

Work within NMC Code of Professional Conduct and scope of practice;

Ensure accurate, timely, and confidential clinical documentation using electronic patient records;

Participate in clinical audit, significant event analysis, and quality improvement activities;

Adhere to safeguarding policies and identify vulnerable patients;

  • Postgraduate diploma or degree in Advanced Practice - accepted for qualifications obtained up to December 2020
  • Qualified Independent Nurse Prescriber, listed on the NMC register
  • Fully compliant with NMC revalidation requirements as outlined in the NMC Revalidation booklet
  • Demonstrates competence and meets the standards expected of a registered ANP working at an advanced level
  • Accredited Minor Illness qualification
  • Experience working in general practice or primary care
  • Excellent clinical, communication, and organizational skills
