Job summary
We are looking for a Clinical Practitioner - Long Term Conditions
This is a new opportunity for an experienced practitioner to lead on long term conditions within the practice.
The Clinical Practitioner will work within the practice team to provide high quality care
which meets the needs of the practices patient population. Working within defined boundaries, they will
be able to make safe clinical decisions relating to assessment, diagnosis,
treatment and review.
Main duties of the job
- See and treat patient with long term conditions
- Triage and treat patients wishing to see a health care
professional, making any necessary referrals to other members of the
primary health care team.
- Ensure clinical practice is safe and effective and remains
within boundaries of competence, and to acknowledge limitations.
- Assess/examine patients, diagnose, plan, treat
and review/evaluate. Assessment may
be face-to-face by appointment or by telephone as part of a triage and
will include patients presenting with an undifferentiated diagnosis. Diagnose and manage patients presenting with
both acute and chronic conditions based on a management plan which
includes a medication plan and other interventions.
- In line with competence and agreed protocols,
prescribe medication, review its effectiveness and make appropriate
adjustments to treatment, ensuring patient understanding of medication
side-effects and interactions, including over-the-counter medication.

Linthorpe Surgery is a well established and respected GP Practice in Middlesbrough. The practice is located on Linthorpe Road and has a branch surgery in North Ormesby. There are 9 GP Partners and a clinical / administrative team of approximately 50.

- A detailed understanding of a General Practice environment
