Job summary

Hillsborough PCN have an exciting opportunity to join their team as the PCN Digital Transformation and Delivery Manager who will work flexibly to support the PCN Clinical Director and the PCN practices to develop the Primary Care Network.

We are looking for someone to work between 30-37.5 hours a week across a minimum of 4 days.

Interviews for the post will take place the morning of 2nd August 2024.

Main duties of the job

You will provide strategic direction and operational management across the PCN; working closely with the PCN Clinical Director and the PCN Board to deliver the requirements of the PCN Network Contract DES.

You will work with a range of healthcare partners and stakeholders and will represent the PCN in a variety of forums to build on the grat work that we have already done.

About us

Hillsborough Primary Care Network (PCN) serves a population of approx. 36,000 patients across the Hillsborough area of Sheffield.

The PCN consists of 4 GP practices Dykes Hall Medical Centre, Far Lane Medical Centre, Tramways Medical Centre and Tramways and Middlewood Medical Centres.

Job responsibilities

The post holder will work with clinical and operational teams within the PCNto create a vision to guide service improvements, identify opportunities to make tangible positive transformation, and provide the inspiration, resource and tools to deliver changes in a collaborative way.

The post holder will plan, coordinate and directly support delivery of transformation activity in general practice within a PCN with a view to improving patient access, staff and patient satisfaction, and the efficiency and sustainability of general practice services.

The post holder will work closely with staff working at practice and PCN level to understand current service models, identify challenges, agree priorities for improvement and then support these to be delivered. The post holder will develop strong relationships with PCN member practices and will support the planning and delivery of improvements that will provide benefits to patients and staff.

  • Educated to degree level or equivalent experience.
  • Experience of working in a role that requires a high degree of autonomy.
  • Ability to prioritise own workload.
  • Experience of working for an NHS organisation at management level.
  • Knowledge and experience of using IT packages.
  • Someone who is highly motivated and enthusiastic.
  • Knowledge and understanding of managing budgets.
  • Exceptional people skills, with a caring compassionate approach, able to both motivate others and have difficult conversations when required.
  • Excellent written and verbal communication skills.
  • Well-developed leadership skills, empowering others to develop.
  • Highly organised, with a keen eye for detail able to prioritise own and others' workloads.
  • Ability to develop and deliver projects.
  • Experience of working with a range of stakeholders and other providers.
  • Experience of managing staff.
  • Ability to work under pressure and deliver deadlines.
