Job summary

We have an exciting opportunity to join our small rural General Practice in Scholar Green. As one of our GP Partners retires this summer, we are looking to recruit a new Advanced Nurse Practitioner working full time or part time in our purpose built modern surgery.

Main duties of the job

The ANP will provide a wide range of health care including treating acute and minor illnesses, chronic diseases, triage and contributing to QOF requirements. The role will support the clinical team seeing a variety of patients with a range of medical conditions face to face, via telephone consultations and home visits. You will also be comfortable analysing and interpreting test results.

The postholder will use skills, knowledge and competencies as a qualified ANP in order to be responsible and accountable for managing patient caseloads.

About us

We are based in South East Cheshire on the border with Staffordshire and are part of the SMASH PCN serving the populations of Sandbach, Middlewich, Alsager, Scholar Green and Haslington. Our Practice has just under 5,000 patients.

Job description

Job responsibilities

Assess, diagnose and treat patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice to patients

Implement and evaluate individual specialised treatment plans for chronic disease patients

Identify, manage and support patients at risk of developing long term conditions, preventing adverse effects on patients health

Provide advanced specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF

Provide assessment, screening services and health education advice

Prescribing and reviewing medication that is appropriate to patient needs and in accordance with the latest evidence based practice.

Develop, implement and embed health promotion and wellbeing programmes

Request diagnostic investigations and interpret within agreed protocols

Maintain accurate clinical records

Prioritise health issues and intervene appropriately

Support the team in dealing with clinical emergencies

Contribute to Practice targets (QOF etc) complying with local and regional guidance

Liaise with external services/agencies to ensure the patient is supported appropriately

Participate in the administrative and professional responsibilities of the Practice team

Support the clinical team with all safeguarding matters

Understand practice and local policies

Maintain up to date skills and knowledge, maintaining awareness of professional issues at an advance level

Qualifications

Essential

  • Registered nurse with Nursing and Midwifery Council
  • Post graduate diploma or degree for advanced practice qualification
  • Qualified independent nurse prescriber on the NMC register
  • Minor illness qualification

Eligibility

  • Meets with NMC revalidation requirements
  • Meets the standards for registered ANP working at advanced level

Other requirements

  • Disclosure Barring Service (DBS) check
  • Occupational Health clearance
