We have an exciting opportunity for a Practice Nurse to join our friendly and dedicated nursing team.
The successful candidate will take a proactive role within the team to provide high standards of nursing care, disease management and health promotion to our Practice population. Delivering specialist chronic disease management to diabetic patients.
This is a part time role, hours are negotiable in the range of 18 - 25 per week.
Main duties of the job
Implement and evaluate individual
treatment plans for chronic disease patients, particularly those with diabetes
Manage routine consultations using a
holistic approach
Health promotion and Long Term Conditions
clinics
Carry
out routine treatment room duties i.e. cervical smears, childhood
immunisations, travel vaccines
Wound care, removal of sutures
ECGs, Venepuncture, INR monitoring (where
HCA availability is limited)
Understand and participate in the
delivery of QOF, local and national Enhanced Services, research and recalls
Support
the clinical team with all safeguarding matters in accordance with local and
national policies
Deliver
opportunistic health promotion where appropriate
Maintain
standards for infection control, hygiene and tidiness
Maintain
accurate records at all times

Key Responsibilities:
