Job summary

We have an exciting opportunity for a Practice Nurse to join our friendly and dedicated nursing team.

The successful candidate will take a proactive role within the team to provide high standards of nursing care, disease management and health promotion to our Practice population. Delivering specialist chronic disease management to diabetic patients.

This is a part time role, hours are negotiable in the range of 18 - 25 per week.

Main duties of the job

Implement and evaluate individual treatment plans for chronic disease patients, particularly those with diabetes

Manage routine consultations using a holistic approach

Health promotion and Long Term Conditions clinics

Carry out routine treatment room duties i.e. cervical smears, childhood immunisations, travel vaccines

Wound care, removal of sutures

ECGs, Venepuncture, INR monitoring (where HCA availability is limited)

Understand and participate in the delivery of QOF, local and national Enhanced Services, research and recalls

Support the clinical team with all safeguarding matters in accordance with local and national policies

Deliver opportunistic health promotion where appropriate

Maintain standards for infection control, hygiene and tidiness

Maintain accurate records at all times

The post holder will take a proactive role within the clinical team to provide high standards of nursing care, disease management and health promotion, as directed by the GPs and in accordance with Practice protocols. Delivering specialist chronic disease management to diabetic patients.

Key Responsibilities:

Experience

Essential

  • Experience in management of diabetic patients, and the initiation of treatment in newly diagnosed diabetic patients, or the willingness to learn
  • Able to manage own workload
  • Experience within Primary Care

Desirable

  • Experience in managing the type of minor illnesses that present in Primary Care

Qualifications

Essential

  • Relevant Nursing Degree / NMC Registration
