Job responsibilities

We are a high Diabetes achieving practice and we are proud to have been consistently in the top 3 practices in Milton Keynes for Diabetes care in the last year, Specifically for completion of the 8 care processes and we were top achievement of "All 3 Treatment Standards". In addition we achieved " Green" for all QOF areas in Diabetes.

You will be well support by both clinical and administrative team who all work very well as a team. You can either focus solely on Diabetes or if you prefer , have a mix of diabetes and other nursing roles if that suits you better.

Working hours are negotiable to help suit your work-life balance.

Diabetes appointments cant be either face to face or telephone consults. (mainly to accommodate patient preferences and facilitate ease of access).

Person Specification

Experience

Essential

• Prior experience and Qualifications in Diabetes is essential, Including experience in additions to and alterations of medications.

Desirable

• Experience of working with A GP Practice

Qualifications

Essential

• Nursing degree
