Job summary

The Advanced Nurse Practitioner (ANP) has been assessed as competent in practice using their expert clinical knowledge and skills. They have the freedom and authority to act and make autonomous decisions in the assessment, diagnosis and treatment of patients.

The ANP provides an opportunity for patients to receive timely care and negate unnecessarily delay in receiving treatment and without the need to refer to a GP.

ANPs in primary care are able to develop close, long-term relationships with their patients and work in partnership with them to achieve optimum health. They are autonomous in making decisions based on assessment, diagnosis and interpretation of test results.

Main duties of the job

Working collaboratively with the General Practitioners, and the wider Primary Health Care Team to support the clinical and wellbeing needs of the patient

Safe and competent clinical decision-making skills must be demonstrable including assessment, diagnosis, treatment and evaluation of care for patients presenting with minor illness within a Primary Care setting.

Provide high quality nursing care from the patients first contact through to resolution or referral as appropriate, in accordance with standards of good practice

To administer and prescribe medication accurately and safely according to established organisational policies and directions, and in accordance with NMC guidelines.

Main duties of the job

A. Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice

B. Implement and evaluate individual specialised treatment plans for chronic disease patients

C. Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patients health

D. Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF

E. Provide wound care

F. Provide specialist clinics such as contraception etc.

G. Undertake the collection of pathological specimens

About us

Our friendly and well established practice has a current partnership made up of , Dr Jatinder Bratch (GP trainer) and Dr Kay Snowdon (GP trainer) and Dr Rohan Shakoor

We have 2 salaried GPs, 2 registrars and a full complement of nurses in our team.

The practice admin team consisting of receptionists, administrators, care coordinators and is led by Sharon Russell, Practice Manager.

The current list size is 8900. We work in Denton Burn in the West of Newcastle and the practice boundaries are restricted to a 2-3 mile radius.

The majority of our patients are in employment, own their own homes, and have a means of transport to the surgery. The demography of our patients is that of an urban practice with a mix of children, young people and the elderly.

We are a SystmOne practice and have a list size of 8900. We are a training team with 2 of the GP Partners being trainers of registrars.

The successful candidate will be available to undertake a variety of duties including telephone, face to face and e-consultations, home visits, medicines management, and associated paperwork.

We are high QOF achievers and rated good by CQC.

Membership of Newcastle Outer West Primary Care Network allows access to First contact practitioners for both mental health and physiotherapists, health and wellbeing team, pharmacists and pharmacy technicians who now carry out many medicines' changes from hospital discharge letters.

Primary Responsibilities

The following are the core responsibilities of the advanced nurse practitioner. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

a. Develop, implement and embed health promotion and well-being programmes

b. Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advise to patients

c. Implement and evaluate individual specialised treatment plans for chronic disease patients

o. Recognise, assess and refer patients presenting with mental health needs

p. Extended and supplementary prescriber, adhering to extant guidance

q. Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required

r. Contribute to practice targets (QOF etc.), complying with local and regional guidance

s. Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.)

t. Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)

u. Support the clinical team with all safeguarding matters, in accordance with local and national policies

v. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
