Job summary
We are seeking an experienced and highly motivated Advanced Nurse Practitioner (ANP) to join our primary care team for 3 days per week.
This is an exciting opportunity for a clinician with strong autonomous practice skills to work within a supportive, multidisciplinary environment. The successful candidate will provide high-quality, patient-centred care, including assessment, diagnosis, treatment, and management of both acute and chronic conditions.
Main duties of the job
The ANP will work independently and collaboratively to deliver a high standard of clinical care. Key responsibilities include:
- Assess, diagnose, plan, implement, and evaluate treatment/interventions for patients presenting with undifferentiated and undiagnosed conditions
- Manage acute presentations, minor illness, and long-term conditions
- Prescribe medications in accordance with local and national guidelines (if qualified as an independent prescriber)
- Order and interpret investigations such as blood tests, imaging, and ECGs
- Work autonomously within scope of practice and professional guidelines
- Provide health promotion, education, and preventative care
- Contribute to practice development, clinical governance, and quality improvement initiatives
- Support and mentor junior staff where appropriate
- Maintain accurate and contemporaneous clinical records

- Request, interpret, and act upon diagnostic investigations

- Contribute to the development and delivery of high-quality, safe, and effective services
- Support implementation of new clinical pathways and service improvements

- Contribute to the development of practice policies and protocols
Multidisciplinary Team Working
- Work collaboratively with GPs, nurses, pharmacists, and wider PCN teams

- Support integrated working across primary, community, and secondary care
Education & Mentorship
- Support and mentor junior staff, students, or trainee ANPs where appropriate
