Job summary

This is an excellent opportunity for a registered Advanced Nurse Practitioner wishing to join Concord Medical Practice.

We have a team of healthcare professionals within the Practice, including GPs, Nurses, ANP, health care assistants, , Care-co-coordinators and clinical pharmacists.

Main duties of the job

The responsibilities will include direct patient contact through assessment, examination, investigation, diagnosis, and treatment. The post-holder will provide holistic clinical services with support from the multi-disciplinary team.

The ANP will work within their scope of clinical practice and provide care for patients presenting with undifferentiated, un diagnosed problems, utilising history-taking, physical examinations, and clinical decision-making skills to establish a working diagnosis and management plan in partnership with the patient.

Working to a professional code of ethics you will be able to demonstrate flexibility in the role and have a calm and caring attitude towards patients.

The successful Advanced Nurse Practitioner will work as an autonomous practitioner, diagnosing, managing, prescribing and discharging patients with illness or injury across all age groups including young children. This may be by face-to-face consultation, telephone or video consultation.

Job responsibilities

The successful post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients.

In accordance with the practice timetable, as agreed, the post-holder will make themselves available to undertake a variety of duties, including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion.

  • An autonomous consultor
  • 10-minute consultations
  • Share of home visits.
  • Able to manage acute presentations
  • Able to manage chronic conditions
  • Able to order appropriate investigations (bloods/radiology).
  • Able to refer appropriately to secondary care or organise emergency admissions.
  • Share of admin: clinical letters which require actioning/filing.

  • ENP/ ECP Minor Injuries and Minor Illness course Level 6

  • Attended X-ray/IRMER course
  • Relevant MSc or BSc
