Job responsibilities
CORE
RESPONSIBILITIES
Each Clinical Pharmacist will
have the following key responsibilities in relation to delivering health
services:
work
as part of a multi-disciplinary team in a patient facing role to clinically
assess and treat patients using their expert knowledge of medicines for
specific disease areas;
be
a prescriber, or completing training to become prescribers, and work with and
alongside the general practice team;
be
responsible for the care management of patients with chronic diseases and
undertake clinical medication reviews to proactively manage people with complex
polypharmacy, especially the elderly, people in care homes, those with multiple
co-morbidities (in particular frailty, COPD and asthma) and people with
learning disabilities or autism (through STOMP Stop Over Medication
Programme);
provide
specialist expertise in the use of medicines whilst helping to address both the
public health and social care needs of patients at the PCNs practice(s) and to
help in tackling inequalities;
provide
leadership on person-centred medicines optimisation (including ensuring
prescribers in the practice conserve antibiotics in line with local
antimicrobial stewardship guidance) and quality improvement, whilst contributing to the quality and
outcomes framework and enhanced services;
through
structured medication reviews, support patients to take their medications to
get the best from them, reduce waste and promote self-care;
have
a leadership role in supporting further integration of general practice with
the wider healthcare teams (including community and hospital pharmacy) to help
improve patient outcomes, ensure better access to healthcare and help manage
general practice workload;
develop
relationships and work closely with other pharmacy professionals across PCNs
and the wider health and social care system;
take
a central role in the clinical aspects of shared care protocols, clinical
research with medicines, liaison with specialist pharmacists (including mental
health and reduction of inappropriate antipsychotic use in people with learning
difficulties), liaison with community pharmacists and anticoagulation; and
be
part of a professional clinical network and have access to appropriate clinical
supervision1
Improve
prescribing practice through educational support for all prescribers within the
practice
Lead
on where changes in evidence require changes in prescribing across patient
population e.g. where a drug is withdrawn or indications change
Liaise
with colleagues in community pharmacy to align support for medicines adherence
such as NMS and DMS
Support
improvements in clinical care through practice based audit and implementing
change
Prescribing
advice to prescribers in practice e.g. temporary non availability of drugs
Ensure
patient safety when they are transferred between care providers through
reconciliation of prescribed medicines
Support
the repeat prescribing reauthorisation process by reviewing requests for repeat
prescriptions and medicines reaching review dates

Demonstrates general pharmaceutical skills and knowledge in core areas
Is able to plan, manage, monitor, advise and review general
pharmaceutical care programmes for patients in core areas, including disease
states / long term conditions identified by local Pharmaceutical Needs
Assessment
Demonstrates accountability for delivering professional expertise and
direct service provision as an individual
Demonstrates ability to use skills in a range of routine situations
requiring analysis or comparison of a range of options
Recognises priorities when problem-solving and identifies deviations from
the normal pattern and is able to refer to seniors or GPs when appropriate

policies/procedures and codes of conduct
Involves patients in decisions about prescribed medicines and supporting
adherence as per NICE guidance

Demonstrates use of appropriate communication to gain the co-operation of
relevant stakeholders (including patients, senior and peer colleagues, and
other professionals, other NHS/private organisations e.g. CCGs)

Is able to recognise personal limitations and refer to more appropriate
colleague(s) when necessary
Liaises with CCG colleagues including CCG Pharmacists on prescribing
related matters to ensure consistency of patient care and benefit

Liaises with other GP Practices and staff as needed for the collective
benefit of patients

Demonstrates understanding of the clinical pharmacist role in governance
and is able to implement this appropriately within the workplace

EDUCATION, TRAINING AND DEVELOPMENT
Enrolment in, or
qualified from an approved 18-month training
pathway or equivalent that equips the Clinical Pharmacist to:
o
be able to practice and prescribe
safely and effectively in a primary care setting (for example, the CPPE
Clinical Pharmacist training pathways); and
o
deliver the core responsibilities
outlined above.
Understands and demonstrates the characteristics of a role model to members
in the team and/or service
Demonstrates understanding of the mentorship process
Demonstrates ability to conduct teaching and assessment effectively
according to a learning plan with supervision from a more experienced colleague
Demonstrates self-development through continuous professional development
activity
Participates in the delivery of education programmes
Demonstrates an understanding of current educational policies relevant to
working areas of practice and keeps up to date with relevant clinical practice

Demonstrates ability to critically evaluate and review literature
