Job summary

An exciting opportunity has arisen for a Nursing Associate to join the Medics PCN. The Medics PCN is a mature, stable and innovative Primary Care Network based in Luton. Medics Primary Care Network consists of 5 friendly practices working together to provide excellent patient care to a population of over 59,000 patients.

You will be a valued member of our large and established multi-disciplinary team (MDT), made up of Clinical Pharmacists, First Contact Physiotherapists, Paramedics, Nursing Associates and a vast personalised care team.

Medics PCN pride themselves on a friendly and supportive team and are looking for a qualified nursing associate to compliment our growing nursing team.

Main duties of the job

The role of the Nursing Associate is to work with the multi professional team to assess, plan, organise and deliver clinical care in line with service and patient needs, as agreed with supervising clinicians.

Main Duties include:

  • Work as part of the PCNs MDTs and communicate proactively and effectively with all MDT colleagues across the PCN, attending and contributing to meetings as required,

  • Support registered nurses and the delivery of nursing care for the practices and the PCN.

  • Perform and record clinical observations such as blood pressure, temperature, respirations, and pulse,

  • Undertaking additional training, provide flu vaccinations, ECGs, and venepuncture, and other relevant clinical tasks as required by the PCN,

  • Maintain accurate and contemporaneous patient health records.

  • Other duties

    Work in adherence to NMC Code for Nursing Associates, within own level of competence and the protocols and policies of the PCN or practice.

    Maintain a good working knowledge of Health and Safety procedures, fire safety and mandatory training as per PCN policies. Participate in policy development and data collection where appropriate.

    Work flexibly to meet the needs of the service and patients. Adapt to what may be potentially challenging and changing situations at times.

    Willing to undertake other duties deemed appropriate to a Nursing Associate following correct training/guidance and agreed by the post holder and their line manager.

Work within the PCN to provide and monitor care, alongside the wider PCN team. contributing to the delivery of excellent patient care, supporting the existing workforce under direct or indirect supervision.

Improve safety and quality of care at every opportunity.

Undertake additional clinical training as required by the PCN to support the Network Contract/DES Services Specification.

Support Advanced Practitioners, Paramedics, Clinical Pharmacists and the wider team to enable them to focus on the more complex clinical care.

To support with chronic disease clinics and maintaining registers, deliver patient care, referring patients to other members of the PCN or to secondary/specialist care as appropriate. To implement and evaluate individual treatment plans dependant on training and experience.

Provide support to the PCN with regards to their Network Contract DES Services Specification including CVD prevention and tackling health inequalities.

Develop relationships across the PCN to support integration of the role across health and social care: including community care, secondary care, and mental health services.

Provide support and supervision to trainee nursing associates, healthcare assistants, apprentices, and those on learning placements appropriate to level of competence.

Accurately perform and record clinical observations such as blood pressure, temperature, respiration, and pulse rates.

Engage positively with patients/carers/families and staff members promoting mutually respectful and appropriate relationships.

Communicate effectively with patients regarding providing and planning their care and carrying out risk assessment needs as required.

Communicate complex and sensitive information verbally and in writing.

Adapt and tailor communication skills to the need of the listener.

To act as patient advocate during PCN meetings, sharing any relevant information confidently and accurately, maintaining patient confidentiality appropriately.

Have an understanding of MDT structures and members roles.

Support patients to improve and maintain their physical, mental, and behavioural health and well-being.

Provide advice to patients including promoting self -management where appropriate and signposting patients to other PCN colleagues, local community, and voluntary sector services.

Maintain accurate and contemporaneous patient health records.
