Job summary

Medics PCN are now advertising for a professional, diligent and dynamic Digital & Transformation Manager to lead their mature and innovative PCN.

Medics PCN Ltd, is a strong and stable Primary Care Network based in Luton, where our 5 practices work together at scale. Medics PCN is an established limited company with the network running for in excess of 5 years. Medics are looking for a new Network Manager to support Dr Barhey, Medics PCN Clinical Director, in the continued success of the PCN.

Medics PCN have a strong team ethos amongst the network team and the Digital & Transformation Manager will be play a key part in supporting this ethic.

Main duties of the job

The post holder will work flexibly to support the PCN Clinical Director and the PCN practices to develop the Primary Care Network. You will provide strategic direction, and at times, operational management across the PCN; working closely with the PCN Clinical Director and the PCN leadership team to achieve success in line with the PCN Network Contract DES and the PCNs local priorities.

The PCN Manager will provide line management to PCN employed staff to ensure they are supported and co-ordinated within the PCN, where required. You will support the strategic direction and management of the PCN alongside the PCN Clinical Director.

You will strive to develop strong working relationships with a wide range of healthcare partners and stakeholders. You will represent the PCN in a variety of forums.

About us

The Medics Primary Care Network (PCN) serves a population of circa 59,000 patients residing across the Luton in Bedfordshire and is a mature and established Limited Company.

Working collaboratively with other partners to strengthen and support the delivery of Primary Care services. The PCN consists of 5 core GP practices Woodland Avenue Practice, Medici Medical Practice, Gardenia & Marsh Farm Practice, Bell House Medical Centre & Barton Hills Medical Group.

Medics PCN have a dedicated network site where many of the MDT, personalised care team and clinicians work collaboratively to deliver a pro-active approach to patient care.

Job responsibilities

Main Duties

Take the lead to oversee the management and delivery of the PCN DES requirements

Work with the PCN Clinical Director to develop and deliver the PCN strategic plans and performance requirements

Develop strategies to progress the overall development of the PCN, with reference to the Maturity Matrix and the PCN Development Plan.

Have a full understanding of the Additional Roles Reimbursement Scheme (ARRS) funded roles and work with the PCN on the planning and deployment of these roles.

To provide line management to PCN employed staff, to ensure that they are embedded in the PCN and co-ordinated across the PCN practices as required.

Co-ordinate all of the PCN meetings, including Board Meetings and Core Team Meetings.

Capture minutes of the core team meetings and share as required.

Liaise with key partners in the development and monitoring of integrated services and projects to develop integrated neighbourhood teams (INT).

Manage the set up and delivery of multiple projects within a programme of work, working with colleagues and subject matter experts to scope, plan, design and deliver those projects.

Support the Clinical Director

Oversee the management and delivery of the PCN DES requirements.

Support the Clinical Director to develop and deliver the PCN development plan and strategic plans, providing performance data as required.

Take an active role in PCN leadership.

Support the Clinical Director with wider system working and INT development.

Support the Clinical Director in having full oversight of incoming and outgoing PCN funds.

PCN Management

Support the PCN and its member practices to formulate project and delivery plans in relation to meeting the DES specification.

Support in the writing of business cases and bids on behalf of the network.

Help to design innovative services that meet local need and provide project management support to help projects progress.

Implement, monitor and report on PCN contracts and services, including project progression and management of risks on behalf of the PCN.

Strategic Management

Working with the PCN Directors to develop and progress the strategic vision.

Support the PCN team to identify, scope and plan improvements.

Deliver quality improvement projects and manage performance to achieve the strategic vision of the PCN.

Seek out examples of good practice and innovation from other areas and present to the PCN as required.

Plan the PCN additional roles workforce, to deliver and achieve the PCN priorities in conjunction with the Clinical Director.

Develop relationships with key partners and stakeholders, exploring opportunities for collaborative and partnership working

Work with BLMK ICB to align strategy and approaches to improvement.

Support staff at all levels in the PCN to take forward change.

Facilitate clinically led innovation and the effective adoption of improvement initiatives, including integrated working at neighbourhood and/or place level to improve access to services for patients.

Have full oversight of PCN finances and support the development of a financial strategy.

HR Duties

Undertake day-to-day line management of PCN staff including management of absences e.g., leave, sickness etc. as required

Oversee the recruitment processes for new PCN staff, from advert to appointment.

Design and create job descriptions, person specifications and other HR documentation as required.

Conduct appraisals and performance management of PCN staff as required.

Support staff to identify and participate in relevant training and professional development opportunities.

Population Health Management

Support a Population Health Management Approach working with place-based stakeholders.

Use data to identify opportunities and to drive improvements in care quality and experience.

Bring together data from a variety of sources and use information to develop insights into the member practices.

Effectively present and communicate data to the PCN core team, board members and other stakeholders when required.

Use data to track and assess impact of PCN DES deliverables and quality improvement projects.

Digital Improvement

Improve the adoption and use of existing and new technology to deliver benefits for patients and staff for the PCN member practices.

Management Admin

Arrange Personal Medical Indemnity for the

PCN Limited Company.

Arrange Limited Liability insurances & other company insurances as required for the PCN Limited Company.

Arrange any additional insurances as required i.e. Cyber policies etc.

Manage and monitor contractual requirements on behalf of the PCN.

Complete National Workforce Reporting Service on a monthly basis.

Personal and Professional Development

Keep abreast of national and local guidance on PCNs.

Keep abreast of other topical and relevant reports to improve understanding and recommendations for PCNs and primary care.

Take responsibility for own development, learning and performance.

Participate in an annual individual performance review, and take responsibility for maintaining a record of own personal and professional development

Review and improve the PCNs digital maturity.

Improve adoption and optimisation of new technology to enhance patient access and improve experience for the PCN practices.
