Job summary

Hambleton North PCN are looking to recruit a Pharmacy Technician to be based at Stokesley Surgery and Great Ayton Health Centre. This post is a PCN role, but the successful candidate will be employed by Stokesley Surgery on behalf of the PCN. This role requires the successful candidate to be registered with the GPhC as a Pharmacy Technician.

Main duties of the job

We are looking to recruit an enthusiastic and dynamic individual who will complement our existing team across both practices.The Pharmacy Technician will play an important role and will work alongside the three Clinical Pharmacists we have working with us already.They will also be supported by the wider clinical team of clinicians including GPs and Advanced Nurse Practitioners.

About us

Both practices are semi-rural, training practices and the teams include GPs, Nurse Practitioners, Physician Associate, Pharmacists, Physiotherapists, Mental Health Practitioner, Nurses, Health Care Assistants, Social prescribers and care co-ordinators. Clinical roles are supported by an excellent administration team. Both practices use Systmone Clinical System.

Job responsibilities

Job Description / Role:

  • Support the role of PCN clinical pharmacists in the delivery of clinical pharmacy services to people in their own homes or local care homes
  • Perform medicines improvement work to maximise safe, cost-effective best practice in prescribing
  • Improve the use of medicines and help patients with medication concordance
  • Assist with medication safety monitoring systems including uncomplicated medication reviews
  • Liaise with hospital teams, local community pharmacies, clinicians and administrative staff to resolve medicine-related queries.
  • Monitor practice prescribing and adherence to practice, local and national prescribing policies
  • Improve service and quality through audit and searches
  • Help identify patients at risk of medicines use issues, including waste, safety and over-use
  • Help manage simple medication requests through liaison with the clinical pharmacist team where appropriate
  • Help to maintain and achieve Quality Outcome Framework targets for both practices

Personal and Professional Development:

The post holder must take responsibility for their development and learning by:

Committing to or currently undertaking the CPPE Primary Care Education Pathway

Meeting GPhC CPD requirements for registration on an annual basis

Completing mandatory training required by the PCN

Ensure principles of openness, transparency and candor are observed and upheld

Have an understanding of GDPR and patient confidentiality

Undergo safeguarding training and adhere to practice safeguarding policies
