Main duties of the job
Key Details:
Start Date:1st September 2025
End Date:31st March 2026
Sessions: Up to 6 per week (flexible session planning available)
The successful candidate will undertake a variety of duties, including:
Face-to-face and telephone consultations
Managing patient queries, reports, prescriptions, and E-consults
Screening for disease risk factors and early detection
Contributing to QOF and enhanced services delivery
Developing care plans collaboratively with patients-Shared, evidence-based prescribing
Accurate, contemporaneous recordkeeping using EMIS.
You will work alongside another GP on-site to ensure support is always available.

As a training practice, we support GP registrars, foundation doctors, and medical students from Sunderland University.

Clinical responsibilities
1.Assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team
2.Implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis
3.Clinically examine and assess patient needs including history taking, assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs and onward refer where appropriate
4.Deliver high-quality healthcare to care home residents who often have complex medical needs, conducting thorough initial assessments upon admission to the care home
5.Use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice
6.Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice
7.Assess, identify, and refer patients presenting with mental health needs in accordance with the NSF for Mental Health Implement and participate in vaccination and immunisation programmes for both adults and children
8.Support the delivery of advanced care plans
9.Actively take a personalised care approach and population centered care approach to enable shared decision making with the presenting person
10.Recognise and work within your own competence and in accordance with professional codes of conduct, maintaining accurate and contemporaneous health records
11.Be aware of your duties and responsibilities regarding current legislation and adhere to our policies and procedures on Safeguarding Children and Safeguarding Adults.
