The Advanced Clinical Nurse Practitioner
will be key to providing excellent patient care to our elderly population and will
be responsible for providing high-quality clinical care, visiting care homes, and
managing frail elderly housebound patients.
The candidate
will possess excellent communication and organisation skills, be approachable,
have a good attitude, and be self-motivated.
Experience in a similar role is desirable but not essential.
Main duties of the job
The purpose of the role is to provide all aspects of care and medication queries to our elderly population and to use various systems of recording, reminding and ensuring that all activities are instigated, monitored, reviewed and completed. Excellent IT skills and high level customer service skills are therefore essential.

Job responsibilities
- Access and triage patients, including same-day triage, and as appropriate provide definitive treatment (including prescibing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team
- Manage undifferentiated, undiagnosed conditions and identify red flags and underlying serious pathology and take appropriate action
- Use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice
- Actively take a personalised care approach and population centred care approach to enable shared decision making with the presenting person
- Complete the relevant training in order to provide multi-professional clinical practice and CPD supervision to other roles within primary care, as required
- Implement and evaluate treatment and interventions within your sphere of competence
- Clinically examine and assess the person's needs, plan care accordingly, using critical thinking and a sound evidence base
- Integrate both drug and non-drug based treatment methods into a management plan, ensuring that appropriate care plans are in place to support the person's treatment
- Act sensitively, confidentially and with empathy in demonstrating understanding of wide-ranging physical and emotional needs. Take the lead in managing contentious, sensitive and delicate issues with patients, relatives and staff
- Develop relationships and a collaborative working approach within TMG, communicating effectively with colleagues, practice staff, Partners, patients and carers where applicable; recognising the need for a variety of communication methods; being confident and competent at interacting at all levels
