Job summary
We are seeking a motivated and patient-focused Clinical Pharmacist to join our friendly and supportive team within our purpose-built practice. The post holder will work as part of the multidisciplinary team to deliver high-quality clinical medication reviews supporting safe, effective, and evidence-based use of medicines.
The role includes undertaking structured medication reviews, managing polypharmacy, supporting patients with long-term conditions, providing expert medicines advice to clinicians and patients, reconciling medicines following hospital discharge, contributing to medicines safety and clinical governance, and responding to medication-related queries from practice staff and patients.

Undertake structured medication reviews and manage polypharmacy to optimise patient outcomes.
Support patients with long-term conditions through medication optimisation and evidence-based therapeutic review.
Provide dedicated clinical pharmacy support to care homes, including undertaking structured medication reviews for care home residents.
Respond to medicines-related queries from care home staff and support safe medicines management within care home settings.
Ensure medicines are safe, appropriate, and effective, identifying and reducing medicines-related risks.
Provide expert, evidence-based medicines advice to GPs, nurses, and members of the multidisciplinary team.
Respond to medication-related queries from practice staff and patients in a timely and professional manner.
Reconcile medicines following hospital discharge and other care transitions to ensure continuity and safety.
Contribute to medicines safety initiatives, clinical audit, and quality improvement activity.
Supervise and support pharmacy technicians as part of the practice pharmacy team.
Maintain accurate and contemporaneous clinical records using EMIS and other IT systems in line with practice policy and professional standards.
Maintain patient confidentiality in accordance with information governance requirements.
