This Advanced Clinical Practitioner role will involve working alongside our multi-disciplinary team which consists of a GP Partners, Advanced Clinical Practitioner Partner and Pharmacist Partner. They are all supported by Advanced Clinical Practitioners who lead on acute same day care, alongside the Home Visiting Team, First Contact Physio, Pharmacists, Practice Nurses and HCAs. We also have Social Prescribers who work as PCN staff dealing with the Mental Health of our patients.
The successful Advanced Clinical Practitioner will work as an autonomous practitioner, diagnosing, managing, prescribing and discharging patients with illness or injury across all age groups including young children. This may be by face-to-face consultation, telephone or video consultation.

Main duties of the job
The Advanced Clinical Practitioner (ACP) has been assessed as competent in practice using their expert clinical knowledge and skills. They have the freedom and authority to act and make autonomous decisions in the assessment, diagnosis and treatment of patients.
The ACP provides an opportunity for patients to avoid the need to be referred to a GP, thereby enabling them to receive timely care and negate unnecessary delay in receiving treatment.
ACPs in primary care can develop close, long-term relationships with their patients and work in partnership with them to achieve optimum health. They are autonomous in making decisions based on assessment, diagnosis and interpretation of test results. They can independently prescribe appropriate medication, evaluate or refer to other specialists if necessary.
