Main duties of the job
As a Practice Nurse at Lower Broughton, you will be a key player in our clinical team. Your primary focus will be:
- Chronic Disease Management:Leading clinics for conditions such as Asthma, COPD, Diabetes, and Hypertension. Being able to assess, implement and evaluate individual treatment plans for patients with a known long-term condition or at risk of a long-term condition
- Cervical Cytology:Performing screening and providing advice on womens health.
- Family Planning and Menopause care: Support and advise patients requesting information relating to family planning needs, and Menopause Health
- General Treatment Room Duties:Including immunisations and travel vaccinations (childhood and adults), and NHS health checks.
- Health Promotion:Empowering patients to manage their own health through education and lifestyle advice.
- Support vulnerable patients with learning disabilities and dementia
- Maintain disease registers
- Support nursing colleagues in maintaining infection control logs.

- Provide general practice nursing services including:
- Management of long-term conditions (e.g. diabetes, asthma, COPD, hypertension)
- Cervical cytology (smear taking)
- Immunisations and vaccinations (including childhood and adult programmes)
- Venepuncture, ECGs, urinalysis, and blood pressure monitoring
- Undertake health promotion and disease prevention activities
- Support patients with lifestyle advice including smoking cessation, weight management, and sexual health
- Assist with minor surgery procedures where required
- Monitor and maintain accurate patient records using our clinical systems EMIS
Professional Responsibilities
- Work within NMC Code of Professional Conduct and maintain professional registration
- Maintain clinical competence through continuous professional development (CPD)
- Participate in audit, quality improvement, and clinical governance activities
- Adhere to infection prevention and control policies
- Ensure safeguarding procedures for children and vulnerable adults are followed
Team & Practice Responsibilities
- Work collaboratively with GPs, healthcare assistants, pharmacists, and other primary care staff
- Contribute to practice meetings and service development
- Support training and supervision of healthcare assistants or student nurses where appropriate
- Assist in achieving QOF and other enhanced service targets

Consideration may be given to nurse applicants who are experienced Community or secondary care nurses, particularly but not exclusively from those with diabetes or respiratory disease experience, who wish to move to primary care with training supported.
Desirable
- Desirable Qualifications: Primary care experience, Independent Prescriber (or willingness to train), Experience using EMIS Web / Docman, Experience of Cervical Cytology & Chronic Disease Management, Proactive approach to QOF targets
