Main duties of the job
Deliver safe, effective, and holistic patient care in accordance with current clinical guidelines.

Manage a wide range of patients, including those with acute and long-term conditions.

Conduct home visits and provide urgent care when required.

Work collaboratively with the wider practice team, including nurses, healthcare assistants, and administrative staff.

Contribute to clinical governance activities, audits, and ongoing service improvement initiatives.

Ensure accurate, comprehensive, and timely completion of all medical records and documentation.

Actively participate in team meetings and practice-wide activities to support a cohesive, patient-centred service.
About us
We are a supportive and successful training
practice of 9,500 patients and you would be joining a robust MDT made up of 5 Partners,
1 salaried GP, 1 GP Retainer, Registrars, Nursing team, Paramedic, Pharmacy
team, as well as our innovative management and admin team.
We work closely with our PCN who provide a
paramedic-led home visiting service for our frail and housebound patients,
meaning there are minimal home visits for our GPs, a first contact physio
service, social prescribing team, and link with mental health and wellbeing
coaches.
