Main duties of the job

  • Deliver safe, effective, and holistic patient care in accordance with current clinical guidelines.

  • Manage a wide range of patients, including those with acute and long-term conditions.

  • Conduct home visits and provide urgent care when required.

  • Work collaboratively with the wider practice team, including nurses, healthcare assistants, and administrative staff.

  • Contribute to clinical governance activities, audits, and ongoing service improvement initiatives.

  • Ensure accurate, comprehensive, and timely completion of all medical records and documentation.

  • Actively participate in team meetings and practice-wide activities to support a cohesive, patient-centred service.

About us

We are a supportive and successful training practice of 9,500 patients and you would be joining a robust MDT made up of 5 Partners, 1 salaried GP, 1 GP Retainer, Registrars, Nursing team, Paramedic, Pharmacy team, as well as our innovative management and admin team.

We work closely with our PCN who provide a paramedic-led home visiting service for our frail and housebound patients, meaning there are minimal home visits for our GPs, a first contact physio service, social prescribing team, and link with mental health and wellbeing coaches.

  • Evidence of ongoing professional development
  • Experience of managing patients in a primary care setting
  • Experience of acute and chronic disease management
  • Experience of preventative care and health promotion
  • Understanding of primary care and the GP contract
  • Awareness of safeguarding procedures for adults and children
  • Knowledge of QOF, Enhanced Services, and national primary care targets
  • Excellent clinical and consultation skills
  • Ability to make sound clinical decisions based on evidence
  • Effective communication and interpersonal skills
  • Good organisational and time management skills
  • Ability to work independently and within a team
  • Commitment to high-quality patient care and safety
