Job summary
Exciting opportunity for an experiencedPractice Nurseto join our friendly general practice. You will undertake all aspects of the practice nurse role including cervical screening, baby immunisations and chronic disease management.
We are a practice serving approx 5000 patients.
You will be joining a team which comprises GPs, Pharmacist and a Physician Assistant, Mental health nurse and physio's. The practice provides patient services to a richly varied, multicultural population.
Experience in General Practice is essential.
Main duties of the job
The Practice Nurse will be responsible for a number of clinical areas such as health promotion, chronic disease management, health prevention, as well as supporting the management team in the reviewing of clinical policies and procedures.
- Initiating and maintaining health screening and promotion clinics for identified areas of the practice population, recognising where modification of systems or referral is necessary.
- Managing the nursing care of patients with chronic disease, employing personal recall systems suitable to the practice needs. This includes using the practice computer systems (We use EMIS web as our main clinical system) for input of information for recall and screening purposes.
